腹部按摩仪联合中医护理对脊髓损伤后神经源性直肠患者排便功能的影响

Effect of Abdominal Massage Device Combined with Traditional Chinese Medicine Nursing on Defecation Function for Patients with Neurogenic Rectum after Spinal Cord Injury

目的:研究腹部按摩仪联合中医护理对脊髓损伤(SCI)后神经源性直肠(NR)患者排便功能的影响。方法:选取本院2017年8月〜2019年11月诊治的SCI后NR患者100例,随机抛硬币法分为对照组(44例)和中医组(56例),前者给予常规直肠功能训练加开塞露灌肠护理,后者在其基础上使用腹部按摩仪联合中医护理。比较两组患者干预前后排便功能、心理健康、排便日记指标和生活质量。结果:腹部按摩仪联合中医组患者排便功能优良率显著高于对照组(P<0.05);干预后沃里克-爱丁堡积极心理健康量表(WEMWBS)评分显著高于对照组,焦虑自评量表(SAS)评分低于对照组(P<0.05);中医组排便时间、粪便性状评分显著低于对照组,排便频率明显高于对照组(P<0.05);中医组患者心理行为、自我感受、窘迫尴尬评分显著高于对照组(P<0.05),但生活方式两组评分比较无统计学意义(P>0.05)。结论:腹部按摩仪联合中医护理应用于SCI后NR可有效促进患者排便功能、性功能的恢复,同时改善患者心理情绪,提高生活质量。

Objective:To study the effect of abdominal massage device combined withTCM nursing on defecation function for patients with neurogenic rectum(NR)after spinal cord injury(SCI).Methods:A total of 100 patients with NRafter SCI who were diagnosed and treated in the hospital from August 2017 to November 2019 were randomized into a controlled group(44 cases)and a TCM group(56 cases).The controlled group was given enema nursing,and on this basis,the TCM group was given abdominal massage device combined with TCM nursing.Defecation function,mental health,defecation diary indexes and quality of life were compared between the two groups before and after intervention.Results:The excellent and good rate of defecation function in the TCM group was significantly higher than that in the controlled group(P<0.05).AfteRintervention,the Warwick-Edinburgh Mental Well-being Scale(WEMWBS)score was significantly higheRthan the controlled group,and the selfrating anxiety scale(SAS)score was lower than the controlled group(P<0.05).The defecation time and fecal characteRscore of TCM group were significantly shorter/lower than those of the controlled group,and the defecation frequency was significantly higher than the controlled group(P<0.05).The psychological behavior,self-perceptionand embarrassment scores of the TCM group were significantly higher than those of the controlled group(P<0.05),but there was no statistical significance in comparison of lifestyle scores between the two groups(P>0.05).Conclusion:The application of TCM nursing combined with abdominal massage device in patients with NRafter SCI can effectively promote the recovery of defecation function and sexual function while improving patients'psychological states and quality of life.
