黄帝内针配合咽部冰刺激对急性缺血性脑卒中并发吞咽障碍患者生活质量的影响

Effect of Huangdi Internal Acupuncture Combined with Pharyngeal Ice Stimulation on Quality of Life for Patients with Acute Ischemic Stroke Complicated with Dysphagia

目的:观察黄帝内针配合咽部冰刺激对急性缺血性脑卒中并发吞咽障碍患者生活质量的影响。方法:采用随机数字表法将84例急性缺血性脑卒中并发吞咽障碍患者分为观察组与对照组,每组各42例。对照组实施常规吞咽训练,观察组在对照组基础上使用黄帝内针配合咽部冰刺激,10d为1个疗程,两组患者均治疗2个疗程。治疗2个疗程后,比较两组患者临床疗效,观察治疗前、治疗2个疗程后两组患者吞咽能力(洼田饮水试验)、生活质量[吞咽相关生存质量量表(SWAL-QOL)]水平变化情况,统计两组患者并发症(窒息、呛咳、吸入性肺炎)发生率。结果:治疗2个疗程后,观察组临床总有效率明显高于对照组(P<0.05);两组患者饮水试验功能评级均优于治疗前,且观察组优于对照组(P<0.05,P<0.01);两组患者SWAL-QOL高于治疗前,且观察组高于对照组(P<0.01);观察组患者并发症发生率低于对照组(P<0.05)。结论:黄帝内针配合咽部冰刺激可显著改善急性缺血性脑卒中并发吞咽障碍患者吞咽功能及生活质量,减少并发症的发生。

Objective:To explore the effect of Huangdi internal acupuncture combined with pharyngeal ice stimulation on quality of life for patients with acute ischemic stroke complicated with dysphagia.Methods:Using random number table method,84 patients with acute ischemic stroke complicated with dysphagia were divided into an observation group and a controlled group,with 42 cases in each group.The controlled group was given routine swallowing training,and the observation group was given Huangdi internal acupuncture combined with pharyngeal ice stimulation on the basis of the controlled group,and the two groups were treated for 2 courses by taking 10d as a course of treatment.After 2 courses of treatment,the clinical efficacy of the two groups was compared,and the swallowing ability(water swallow test)and quality of life[Swallowing Related Quality of Life Scale(SWALQOL scale)]were observed in the two groups before treatment and after 2 courses of treatment,and the incidence rates of complications(asphyxia,bucking,aspiration pneumonia)were counted in the two groups.Results:After 2 courses of treatment,the total clinical effective rate in the observation group was significantly higher than that in the controlled group(P<0.05).The grade of water swallow test function of the two groups was better than that before treatment,and the grade in the observation group was superior to that in the controlled group(P<0.05,P<0.01).The SWALQOL of the two groups was higher than that before treatment,and the score in the observation group was higher than that in the controlled group(P<0.01).The incidence rate of complications in the observation group was lower than that in the controlled group(P<0.05).Conclusion:Huangdi internal acupuncture combined with pharyngeal ice stimulation can significantly improve the swallowing function and quality of life,and reduce the occurrence of complications of patients with acute ischemic stroke complicated with dysphagia.
