α-synuclein蛋白及LRRK2在帕金森患者血清中的表达及临床研究

Expression and clinical value ofα-synuclein and LRRK2 in serum of Parkinson’s disease

目的研究α-突触核蛋白(α-synuclein)、富亮氨酸重复激酶2(leucine-rich repetitive kinase2,LRRK2)在帕金森病(Parkinson’s disease,PD)患者血清中的表达及临床价值研究。方法选取2016年1月-2018年12月皖南医学院弋矶山医院收治的PD患者90例,根据是否腔隙梗死将患者分为腔梗患者25例,无腔梗患者65例;根据空腹外周血高同型半胱氨酸(hyperhomocysteine,Hhcy)将患者分为:Hhcy血症患者29例,正常Hcy患者61例;按Hoehn-Yahr(H-Y)分级修正量表将患者分为:PD1-2级36例、PD2.5-3级34例、PD4-5级20例。另外选取同期在本院进行健康体检的健康老人90例。对比两组研究对象的一般资料、LRRK2、α-synuclein表达水平,并对PD患者各个亚组进行比较,分析LRRK2、α-synuclein表达水平和PD患者严重程度的关系,采用Pearson分析LRRK2、α-synuclein之间的相关性。结果健康对照组、PD两组研究对象性别、年龄、体质量比较,差异无统计学意义(P>0.05);PD组的LRRK2、α-synuclein表达水平均高于健康对照组,与无合并高血压患者相比,合并高血压患者LRRK2、α-synuclein水平较高(P<0.05)。与无腔隙梗死患者相比,合并腔隙梗死患者LRRK2、α-synuclein水平较高,差异有统计学意义(P<0.05)。与无Hhcy血症患者相比,Hhcy血症患者LRRK2、α-synuclein水平较高(P<0.05)。与12级相比,2.53级、4-5级LRRK2、α-synuclein表达水平升高(P<0.05);与2.5-3级相比,4-5级LRRK2、α-synuclein表达水平升高(P<0.05)。LRRK2、α-synuclein之间均呈正相关(r=0.246,P=0.019)。结论LRRK2、α-synuclein在PD患者血清高表达,且与患者严重程度有关,为临床PD的诊断提供参考。

Objective To study the expression and significance of leucine-rich repetitive kinase 2(LRRK2)andα-synuclein in Parkinson’s disease(PD).Methods From January 2016 to December 2018,90 patients with PD admitted to our hospital were selected.90 patients were divided into 25 patients with lacunar infarction and 65 patients with no lacunar infarction.According to fasting peripheral blood homocysteine(Hey),29 patients with high homocysteine(Hhcy),61 patients with normal Hey,36 patients PD1-2 grade,34 patients PD2.5-3 grade and 20 patients with PD4-5 grade.In addition,90 healthy elderly patients who underwent physical examination in our hospital at the same period were selected as the health control group.The general data,LRRK2 and alpha-synuclein expression levels of the two groups were compared,and PD were divided according to the severity of the disease.The relationship between LRRK2 andα-synuclein expression levels and the severity of PD was analyzed and compared.Pearson was used to analyze the correlation between LRRK2 andα-synuclein.Results There was no significant difference in sex,age and weight between healthy control group and PD group(P>0.05).The expression level of LRRK2 andα-synuclein in PD group was higher than that in healthy control group,and the level of LRRK2 andα-synuclein in patients without hypertension was lower than that in patients with hypertension(P<0.05).Compared with patients with no lacunar infarction,patients with lacunar infarction had higher levels of LRRK2 andα-synuclein(P<0.05).Compared with patients with normal Hcy,patients with higher Hcy had higher levels of LRRK2 andα-synuclein(P<0.05).Compared with level 1-2,the expression levels of LRRK2 andα-synuclein in level 2.5-3,4-5 were significantly higher(P<0.05).Compared with level 2.5-3,the expression levels of LRRK2 andα-synuclein in level4-5 were significantly higher(P<0.05).Conclusion LRRK2 andα-synuclein are highly expressed in PD patients.With the severity of Parkinson’s stage,LRRK2 andα-synuclein gradually increase.LRRK2 andα-synuclein are positively correlated and play an important role in PD development.
