2015~2019年曲阜市居民死因监测分析

目的了解曲阜市2015~2019年居民死亡率及其特征,分析居民死亡率的变化趋势。方法曲阜市户籍人口死亡资料来源于曲阜市居民死因登记报告信息系统。户籍人口资料由曲阜市公安局提供。死因数据统一采用《国际疾病分类方法》(ICD-10)进行编码。采用Excel 2013和SPSS 21.0软件对死亡个案进行数据整理和描述性分析。结果 2015~2019年曲阜市年均粗死亡率为706.78/10万,年均标化死亡率为523.57/10万。近5年总人群总死因前5位分别是心脏病(242.45/10万)、恶性肿瘤(174.85/10万)、脑血管疾病(146.46/10万)、呼吸系统疾病(47.55/10万)、伤害(41.39/10万),前5位死因占全死因的92.18%。居民潜在寿命损失前3位的死因依次是恶性肿瘤、心脏病、脑血管疾病,PYLL依次为13 232.38、7 488.84、7 155.40人年。导致平均寿命损失前3位的死因依次是先天异常、产科疾病、伤害,AYLL依次为33.81、26.32、23.57年/人。以上结果差异均有统计学意义(P<0.01)。结论从2017年开始,曲阜市居民年均标化死亡率,有逐年上升的趋势,期望寿命略有下降。恶性肿瘤、循环系统疾病等慢性非传染性疾病仍是影响当地居民健康的主要病因。

Objective To understand the residents’ mortality and its characteristics in Qufu city during 2015-2019,and analyze the changing trend of residents’ mortality. Methods The death data of registered population in Qufu city came from Qufu city Resident Death Registration and Reporting Information System.The registered population data were provided by Qufu municipal public security bureau.Cause-of-death data were coded using the International Classification of Diseases(ICD-10).Excel 2013 and SPSS 21.0 were used for data compilation and descriptive analysis of the death cases. Results The average crude mortality in Qufu city during 2015-2019 was 706.78/105,and the annual average standardized mortality was 523.57/105.In the past 5 years, the top 5 causes of death were heart disease(242.45/105),malignant tumor(174.85/105),cerebrovascular disease(146.46/105),respiratory disease(47.55/105),injury(41.39/105),and the top 5 causes accounted for 92.18% of the total causes of death.The top 3 causes of death for potential life loss of residents were malignant tumor, heart disease and cerebrovascular disease in sequence, and PYLL was 13 232.38,7 488.84 and 7 155.40 person-years in sequence.The top 3 causes of life expectancy loss were congenital abnormality, obstetrical disease and injury in sequence, and AYLL was 33.81,26.32 and 23.57 years/person in sequence.The differences of the above results were statistically significant(P<0.01). Conclusion Since 2017,the average annual standard mortality of Qufu residents has increased year by year, and the life expectancy has decreased slightly.Chronic non-communicable disease such as malignant tumor and circulatory system disease were still the main causes affecting the health of local residents.
