2018~2020年汝南县艾滋病患者 CD4+T淋巴细胞数量变化与感染情况调查

摘要 目的探讨艾滋病患者CD4+T淋巴细胞的表达及意义。方法选取2018年9月至2020年9月于汝南县疾病控制中心艾防科接受治疗的85例艾滋病患者,统计患者病原微生物感染情况,且所有患者均接受CD4+T淋巴细胞水平检测。结果 85例艾滋病患者中,75例患者发生感染,占88.24%;10例患者未发生感染,占11.76%。75例感染患者中,口腔假丝酵母菌感染20例;其次为细菌性肺炎18例;巨细胞病毒感染最低2例;CD4+T淋巴细胞水平≤50(个/μl)患者感染率最高,占52.00%;其次为CD4+T淋巴细胞为51~100(个/μl)患者,占33.33%;CD4+T淋巴细胞≥211(个/μl)患者感染率最低,占2.67%;CD4+T淋巴细胞≤50(个/μl)中,口腔假丝酵母菌感染感染率高于其他感染类型,差异有统计学意义(P<0.05)。CD4+T淋巴细胞为51~100(个/μl)中,感染性腹泻感染率高于其他感染类型,差异有统计学意义(P<0.05)。CD4+T淋巴细胞为101~210(个/μl)、≥211(个/μl)中各个感染类型发生率相比,差异无统计学意义(P>0.05)。结论汝南县艾滋病患者病原微生物感染发生率较高,其中以口腔假丝酵母菌、细菌性肺炎表现尤为明显,且患者CD4+T淋巴细胞水平越低,感染概率越高,临床可根据该细胞水平变化,积极采取防治措施,以期降低艾滋病患者感染发生率。 Objective To explore the expression and significance of CD4+T lymphocytes in AIDS patients. Methods From September 2018 to September 2020,85 AIDS patients who were treated in the department of preventive medicine, Runan county center for disease control and prevention were selected, and the infection of pathogenic microorganisms was counted.All patients were tested for CD4+T lymphocyte level. Results Among the 85 AIDS patients, 75 patients were infected, accounting for 88.24%.No infection occurred in 10 patients, accounting for 11.76%;Among the 75 infected patients, 20 were infected with oral candida.The second was bacterial pneumonia in 18 cases.The lowest number of patients infected with cytomegalovirus was 2.The infection rate of patients with CD4+T lymphocyte level ≤50(/μl) was the highest, accounting for 52.00%.Secondly, patients with CD4+T lymphocyte level of 51-to 100(/μl),accounting for 33.33%;the infection rate of patients with CD4+T lymphocyte level≥211(/μl) was the lowest, accounting for 2.67%.When CD4+T lymphocyte level ≤50(/μl),oral Candida infection rate was higher than other infection types, and the difference was statistically significant(P<0.05).When CD4+T lymphocyte level was 51-100(/μl),the infection rate of infectious diarrhea was higher than that of other infection types, and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of infection types between 101-210(/μl) and ≥211(/μl) levels of CD4+T lymphocytes(P>0.05). Conclusion The incidence of pathogenic microorganism infection in AIDS patients in Runan county is high, especially oral candida and bacterial pneumonia.The lower the level of CD4 + T lymphocyte, the higher the infection probability.According to the change of the level of CD4 + T lymphocyte, it is necessary to actively take preventive measures to reduce the incidence of infection in AIDS patients.
