超声评价产前胎儿胰腺结构特点、发育状况及其随孕周变化的研究

Study of prenatal ultrasound in evaluating the characteristics of pancreas structure,development situation and changes of them with gestational age of fetus

目的:分析超声在评价产前胎儿胰腺结构特点、发育状况及其随孕周变化中的价值。方法:选取唐山市妇幼保健院经产前超声检查的346例孕产妇,采用超声诊断仪观察胎儿各系统发育情况及附属物情况。测量胎儿胰腺周长,以及胰头、胰体和胰尾厚度,采用Pearson分析法分析胎儿胰腺周长,以及胰头、胰体和胰尾厚度与胎儿腹围、孕周的相关性。分析超声在评价胎儿胰腺异常中的价值。结果:在346例单胎孕妇中,超声显示胎儿完整胰腺共216例,整体满意的显示率为62.43%。216例完整胰腺胎儿情况的胰腺周长,胰头、胰体及胰尾厚度随着孕周的增加逐渐增大。Pearson分析结果显示,胎儿胰腺周长,胰头、胰体及胰尾厚度与胎儿腹围呈明显正相关(r=0.983,r=0.720,r=0.565,r=0.674;P<0.05),与孕周亦呈明显正相关(r=0.913,r=0.715,r=0.636,r=0.589;P<0.05)。结论:超声对产前胎儿胰腺异常的检查具有可行性,胰腺周长、厚度与孕周、腹围呈明显正相关,胎儿胰腺周长、厚度随孕周的变化参考值可为产前诊断先天性胰腺疾病、肠道梗阻等疾病提供依据。

Objective:To analyze the value of prenatal ultrasound in evaluating the characteristics of pancreas structure,development situation and changes with gestational age of fetus.Methods:346 pregnant women who underwent prenatal ultrasound examination in Tangshan maternal and child health hospital were selected.The development of fetal various system and appendages were observed by ultrasound diagnostic instrument.The pancreatic circumference,and the thicknesses of pancreatic head,pancreatic body and pancreatic tail were measured.Pearson analysis was used to analyze the correlations between fetal pancreatic circumference,the thicknesses of pancreatic head,pancreatic body and pancreatic tail and the abdominal circumference and gestational age.And then,the value of ultrasound in evaluating pancreas abnormalities of fetus was further analyzed.Results:In 346 pregnant women with single fetus,the ultrasound showed 216 cases had complete pancreas,and the display rate of overall satisfactory was 62.43%.The results showed that the pancreatic circumference and the thicknesses of pancreatic head,pancreatic body and pancreatic tail gradually increased with the increasing of gestational age.Pearson analysis showed that fetal pancreatic circumference and the thicknesses of pancreatic head,pancreatic body and pancreatic tail were significantly positive correlation with abdominal circumference of fetus(r=0.983,r=0.720,r=0.565,r=0.674,P<0.05).And they were also significantly positive correlation with gestational age(r=0.913,r=0.715,r=0.636,r=0.589,P<0.05).Conclusion:Prenatal ultrasound is feasible for the examination of pancreas abnormalities of fetus.The circumference and thickness of pancreas are significantly positive correlation with gestational age and abdominal circumference.The changes of the parameters of circumference and thickness of pancreas with gestational age can provide basis for prenatal diagnosis of congenital disease of pancreas,intestinal obstruction and other diseases.
