呼吸心跳骤停行CPR抢救成功的影响因素及干预对策

Investigation on the factors influencing the success of cardiopulmonary resuscitation in patients with respiratory and cardiac arrest

目的分析影响呼吸心跳骤停行心肺复苏(CPR)抢救成功的因素,并探讨其干预对策。方法回顾性分析2016年1月至2021年1月在南方医科大学深圳医院急诊科行CPR抢救的60例呼吸心跳骤停患者的临床资料,其中抢救成功者22例,失败者38例。采用单因素及Logistic多元回归分析影响呼吸心跳骤停行CPR抢救成功的因素。结果单因素分析结果显示,影响CPR抢救成功的因素与性别、年龄无关(P>0.05),与CPR开始时间、复苏地点、电击除颤、心源性疾病、肾上腺素用量、插管时间有关(P<0.05);经Logistic多元回归分析结果显示,CPR开始时间、插管时间、肾上腺素用量、心源性疾病、电击除颤、复苏地点为CPR救治成功的独立危险因素(P<0.05)。结论出现呼吸心跳骤停的患者行CPR抢救会受较多因素影响,应及早采取规范的救治措施,提升救治成功率。

Objective To analyze the factors influencing the success of cardiopulmonary resuscitation(CPR) in patients with respiratory and cardiac arrest.Methods The clinical data of 60 patients with respiratory and cardiac arrest who underwent CPR in the Emergency Department at Shenzhen Hospital,Southern Medical University from January 2016 to January 2021 were retrospectively analyzed.Among them,22 patients were successfully rescued and 38 patients were failed.Univariate and Logistic regression analysis were used to analyze the factors influencing the success of CPR in patients with respiratory and cardiac arrest.Results Univariate analysis showed that the factors influencing the success of CPR rescue were not related to gender and age(P>0.05),but related to the start time of CPR,place of resuscitation,defibrillation,cardiogenic diseases,dosage of epinephrine and intubation time(P<0.05).Logistic multiple regression analysis showed that the start time of CPR,intubation time,dosage of epinephrine,cardiogenic diseases,defibrillation,and resuscitation site were independent risk factors for successful CPR treatment(P<0.05).Conclusion CPR rescue for patients with respiratory and cardiac arrest will be affected by many factors.Standardized treatment measures should be taken as soon as possible to improve the success rate of treatment.
