PPAR⁃γ、MMP⁃3及Bcl6水平变化与RA病情活动性和病理评分的相关性 被引量:5

摘要 目的探讨类风湿性关节炎(RA)患者过氧化物酶体增殖活化受体⁃γ(PPAR⁃γ)、基质金属蛋白酶3(MMP⁃3)及B细胞淋巴瘤因子6(Bcl6)的表达水平,分析上述指标对诊断RA及评估病情活动性的价值。方法选取2019年8月至2020年8月本院收治的102例RA患者为研究组,根据RA患者的病情评分(Disease activity score 28 ,DAS28)分为低活动度组(n=29)、中活动度组(n=45)、高活动度组(n=28),选取同期于本院体检的102例健康志愿者为对照组。比较各组PPAR⁃γ、MMP⁃3及Bcl6表达水平,分析PPAR⁃γ、MMP⁃3及Bcl6与RA患者病情活动程度及滑膜炎病理评分的相关性。结果研究组PPAR⁃γ、MMP⁃3水平及Bcl6阳性表达率均高于对照组,差异具有统计学意义(P<0.05)。PPAR⁃γ、MMP⁃3水平及Bcl6阳性表达:重活动度组>中活动度组>低活动度组,差异具有统计学意义(P<0.05)。Spearman相关分析显示,PPAR⁃γ、MMP⁃3及Bcl6与RA患者病情活动程度及滑膜炎病理评分均呈正相关(P<0.05)。结论PPAR⁃γ、MMP⁃3及Bcl6水平可用于RA病情活动性及滑膜病理情况判断,可作为临床诊断RA和评估病情的辅助检查。 Objective To explore the expression of peroxisome proliferator⁃activated receptor⁃γ(PPAR⁃γ),matrix metalloprotein⁃3(MMP⁃3)and B cell lymphoma(Bcl)6 in patients with rheumatoid arthritis(RA)and to analyze the value of the above indicators in the diagnosis of RA and in the evaluation of disease activity.Methods The clinical data of 102 RA patients admitted to this hospital from August 2019 to August 2020 were selected as the study group.According to the disease activity score 28(DAS28)of RA patients,the group was further divided into the low activity group,the medium activity group,and the high activity group.102 healthy people who had examined in this hospital during the same period were selected as the control group.The expression levels of PPAR⁃γ,MMP⁃3 and Bcl6 in different groups were compared.The correlations between the expression levels of PPAR⁃γ,MMP⁃3 and Bcl6,the degree of disease activity in RA patients and the pathological score of synovitis were analyzed.Results The levels of PPAR⁃γand MMP⁃3 and the positive expression rate of Bcl6 in the study group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The levels of PPAR⁃γ,MMP⁃3 and the positive expression rate of Bcl6 were as follows:the high activity group>the medium activity group>the low activity group,and the difference was statistically significant(P<0.05).Spearman correlation analysis showed that PPAR⁃γ,MMP⁃3 and Bcl6 were positively correlated with the activity level of RA patients and the pathological score of synovitis(P<0.05).Conclusion The levels of PPAR⁃γ,MMP⁃3 and Bcl6 can be used to judge the disease activity of RA and the pathological state of the synovium,and can be used as an auxiliary examination for clinical diagnosis of RA and assessment of the disease.
