成人恒牙根尖发育不全根尖屏障术治疗预后影响因素分析

Risk factors for poor prognosis of adult permanent teeth after apical barrier surgery

目的:分析成人恒牙经根尖屏障术治疗后预后不良的危险因素。方法:分析2016年1月-2017年12月南方医科大学附属龙华人民医院口腔科收治的98颗根尖发育不全的成人恒牙,均实施根尖屏障术,术后6、12、24个月分别评估临床疗效。多因素logistic回归分析预后不良的危险因素。结果:单因素分析显示术后24个月内年龄、根尖孔直径、根尖透射影直径、超填、根管暴露、再治疗等因素与疗效有显著相关性;多因素logistic回归分析显示术后6个月根尖透射影OR值为4.945,术后12个月根尖透射影OR值为9.218,根管暴露OR值为179.183;术后24个月根尖透射影OR值为2.976(P<0.05)。结论:根尖透射影、根管暴露是成人根尖屏障术术后2年内预后不良的危险因素。

Objective:To analyze the risk factors for poor prognosis of adult permanent teeth after apical barrier surgery.Methods:Totally 98 adult permanent teeth with open apex treated in the Department of Stomatology, Longhua people’s Hospital Affiliated to Southern Medical University from January 2016 to December 2017 were retrospectively analyzed.All teeth were carried out apical barrier surgery.The clinical efficacy at 6,12 and 24 months after surgery were analyzed and evaluated.Multivariate logistic regression was used to analyze the risk factors of poor prognosis.Results:Univariate analysis showed that age, diameter of apical foramen, diameter of apical lesion, overfilling, root canal exposure and retreatment within 24 months after operation were significantly correlated with the curative effect. Multifactorial logistic regression analysis showed an OR of 4.945 for apical lesion at 6 months postoperatively, OR of 9.218 for apical lesion and 179.183 for root canal exposure at 12 months postoperatively, OR of 2.976 for apical lesion at 24 months postoperatively(P<0.05).Conclusion:Apical lesion and root canal exposure are risk factors for poor prognosis within 2 years after apical barrier surgery in adults.
