床旁肺部超声在新生儿肺炎中的诊断价值

Diagnostic value of bedside pulmonary ultrasound in neonatal pneumonia

目的:探讨床旁肺部超声在新生儿肺炎中的诊断价值。方法:选择2020年7月—2020年12月江西省赣州市会昌县人民医院收治的新生儿肺炎患儿62例,分析患儿临床资料、肺部超声资料,根据新生儿肺炎诊断的金标准,对临床确诊为新生儿肺炎的病例进行超声声像图像分析。记录患儿的基本情况、肺部超声图像,分析肺泡间质综合征、异常胸膜线、肺实变、肺滑动征消失、胸腔积液、肺不张、肺表面活性物质(PS)使用、呼吸机使用、急性呼吸窘迫综合征(ARDS)、胸腔积液、气胸、肺不张、早发性感染、晚发性感染等情况。结果:62例患儿中肺泡间质综合征42例,发病率为67.74%;异常胸膜线41例,发生率为66.13%。使用呼吸机的肺炎新生儿肺部超声声像图各指标发生率高于未使用呼吸机的肺炎新生儿,差异有统计学意义(P<0.05)。结论:床旁肺部超声在新生儿肺炎中的诊断价值较高,可以辅助新生儿肺炎的临床诊治。

Objective:To explore the diagnostic value of bedside pulmonary ultrasound in neonatal pneumonia.Methods:Sixty-Two cases of neonatal pneumonia were selected from July to December 2020 in Huichang People’s Hospital of Ganzhou City,Jiangxi Province,the clinical data and pulmonary ultrasound data were analyzed,and according to the gold standard for the diagnosis of neonatal pneumonia,ultrasound imaging and image analysis were performed on the clinically diagnosed cases of neonatal pneumonia.The children’s basic information,and lung ultrasound images were recorded to analyze alveolar interstitial syndrome,abnormal pleural lines,lung consolidation,disappearance of lung sliding signs,pleural effusion,atelectasis,pulmonary surfactant(PS)use,breathing machine use,acute respiratory distress syndrome(ARDS),pleural effusion,pneumothorax,atelectasis,early-onset infection,late-onset infection,etc.Results:In 62 cases,42 cases were alveolar interstitial syndrome,and the incidence rate was 67.74%;there were 41 cases of abnormal pleura,and the incidence was 66.13%.The incidence rate of each index of lung ultrasound sonogram of newborns with ventilator pneumonia was higher than that of newborns without ventilator pneumonia,and the difference was statistically significant(P<0.05).Conclusion:Bedside pulmonary ultrasound has a high diagnostic value in neonatal pneumonia and can assist the clinical diagnosis and treatment of neonatal pneumonia.
