使用速效救心丸患者肾功能异常影响因素的真实世界研究

Real World Study on the Influencing Factors of Renal Dysfunction in Patients Treated with Suxiao Jiuxin Pills

目的探究真实世界中使用速效救心丸的患者出现肾功能异常的影响因素,为临床合理使用速效救心丸、降低风险提供一定参考依据。方法从37家三甲医院HIS系统选取了2001年至2015年收录的住院病例,回顾性分析使用速效救心丸的患者临床用药数据信息(23575例)。本研究数据使用SMOTE抽样和Logis⁃tic回归分析相结合的统计学方法处理。结果合并病为慢性肾功能不全,合并使用呋塞米、硝酸甘油可能是发生肾功能异常的影响因素,结果具有统计学意义(P<0.01)。结论速效救心丸在合并病为慢性肾功能不全,合并用药为呋塞米、硝酸甘油使用时应定期检测肾功能,如果发现患者的肾功能指标异常应立刻处理,以保障患者生命健康安全。

Objective:To explore the influencing factors of renal function abnormalities in patients using Suxiao Jiuxin Pills in the real world,and provide a certain reference for the rational use of Suxiao Jiuxin Pills in clinical practice to reduce the risk of abnormal renal function.Methods:Inpatient cases collected from 2001 to 2015 were selected from the HIS system of 37 tertiary A hospitals in China,and clinical data of patients(23575 cases)using Suxiao Jiuxin Pills were retrospectively analyzed.SOMTE sampling and Logistic regression were used for data analysis.Results:It was found that the use of Suxiao Jiuxin Pills combined with chronic renal insufficiency or the combination of Furosemide or Nitroglycerin were the influencing factors of renal dysfunction.The result was statistically significant(P<0.01).Conclusion:Patients using Suxiao Jiuxin Pills should be monitored for renal function when the combined disease is chronic renal insufficiency and the combined medication is Furosemide or Nitroglycerin.Once abnormalities are found,they should be handled in time to ensure the safety of clinical medication.
