整颈“三步九法”联合卧位多角度牵引治疗神经根型颈椎病的临床研究

Clinical Study on the Treatment of Cervical Spondylotic Radiculopathy with“Three Steps and Nine Methods”Combined with Multi-angle Traction in Lying Position

目的观察整颈“三步九法”联合卧位多角度牵引治疗神经根型颈椎病(CSR)的临床疗效。方法54例CSR患者采用随机分配的方法分为研究组与对照组各27例。研究组整颈“三步九法”联合卧位多角度牵引治疗,对照组给予中药热敷联合中立位牵引。7 d为1疗程,两组患者均连续治疗2个疗程。治疗后对比两组患者在总有效率、视觉模拟量表(VAS)评分、颈椎曲度指数D值、颈椎功能障碍指数(NDI)评分、颈椎病临床评价量表(CASCS)评分方面的差异。结果研究组的总有效率为96.30%,高于对照组的85.19%(P<0.01)。两组患者经2周治疗后VAS评分、颈椎曲度指数、NDI评分较前均明显降低(P<0.01),CASCS评分较前均明显升高(P<0.01),研究组的改善情况更优(P<0.01)。两组患者在诊疗期间均未有不良反应的发生。结论整颈“三步九法”联合卧位多角度牵引疗效确切,可明显降低VAS评分、颈椎曲度指数D值、NDI评分、提高CASCS评分。

Objective:To observe the clinical efficacy of“three steps and nine methods”combined with multiangle traction in lying position for the treatment of cervical spondylotic radiculopathy(CSR).Methods:A total of 54 CSR patients were randomly assigned to the study group and control group,27 cases in each group.The study group was treated with“three steps and nine methods”combined with multi-angle traction in lying position for the whole neck,and the control group was given traditional Chinese medicine hot compress combined with neutral traction.Seven days was a course of treatment,and both groups of patients were treated for 2 consecutive courses.After treatment,the differences in total effective rate,visual analog scale(VAS)score,cervical curvature index D value,NDI score,and CASCS score were compared between the two groups.Results:The total effective rate of the study group was 96.30%,which was higher than 85.19%of the control group(P<0.01).After 2 weeks of treatment,the VAS score,cervical curvature index,and NDI index of the two groups were significantly lower than before(P<0.01),and the CASCS score was significantly higher than before treatment(P<0.01).The improvement of the study group was better(P<0.01).There were no adverse reactions in the two groups during the course of diagnosis and treatment.Conclusion:“Three steps and nine methods”combined with multi-angle traction for the whole neck has a definite effect.It can significantly reduce the VAS score,the cervical curvature index D value,the cervical spine dysfunction index,and improve the CASCS score.
