升陷通络法治疗胸闷变异性哮喘的探讨 被引量:5

摘要 以胸闷变异性哮喘的临床特点为依据,运用张锡纯“大气理论”和“络病理论”指导胸闷变异性哮喘的辨证论治。胸闷变异性哮喘总的病机为大气下陷、心肺络滞。大气下陷为胸闷变异性哮喘病机的核心,心肺络滞为胸闷变异性哮喘病机的本质。治以升陷通络法,“升陷法”与“通络法”并行不悖,相辅相成。“肺络郁滞”体现出胸闷变异性哮喘与典型支气管哮喘的共性;而“大气下陷”和“心络郁滞”彰显了胸闷变异性哮喘与典型支气管哮喘在中医病因病机及治疗上的区别。升陷通络法体现中医辨证论治的优势,丰富了胸闷变异性哮喘的中医诊治思路。 Based on the clinical characteristics of chest tightness variant asthma,Zhang Xichun′s academic thought of“pectoral qi theory”and“collateral disease theory”were used to guide the syndrome differentiation and treatment of chest tightness variant asthma.The general pathogenesis of chest tightness variant asthma was pectoral qi subsidence,cardiopulmonary collaterals stagnation.Pectoral qi subsidence was the core of chest tightness variant asthma,and cardiopulmonary collaterals stagnation was the essence of chest tightness variant asthma.In the treatment,the“ascending subsidence method”and the“dredging collaterals method”were compatible and complementary to each other.“Lung collaterals stagnation”reflected the commonness of chest tightness variant asthma and typical bronchial asthma;“pectoral qi subsidence”and“heart collaterals stagnation”highlighted the differences between chest tightness variant asthma and typical bronchial asthma in the etiology,pathogenesis and treatment of TCM.The method of ascending subsidence and dredging collaterals embodied the advantages of TCM syndrome differentiation and treatment,enriched the TCM diagnosis and treatment ideas of chest tightness variant asthma.
