伤科黄水外敷治疗急性期肩周炎的疗效观察

Eeffects of Shangke Huangshui Gauze External Application on Acute Scapulohumeral Periarthritis

目的观察伤科黄水外敷治疗急性期肩周炎的临床疗效并探讨其作用机制。方法124例患者按随机数字表法分为对照组与治疗组各62例。对照组患者给予常规治疗,治疗组患者在此基础上联合使用伤科黄水外敷。分析比较两组患者的临床疗效,治疗前后的视觉模拟疼痛评分法(VAS)、Constant Murley肩关节功能评分(CMS)、肩关节活动度,记录治疗后的整体印象改善评分(CGI-S),疼痛和活动障碍缓解时间。结果治疗组总有效率96.77%,高于对照组的87.10%(P<0.05)。两组患者治疗后VAS评分均较治疗前降低,且治疗组治疗后VAS评分低于对照组(均P<0.05)。两组治疗后患者日常活动能力、肩关节活动度、肌力、疼痛评分、后伸、前屈、外展、内收、内旋、外旋角度等均较治疗前升高,且治疗组治疗后日常活动能力、肩关节活动度、肌力、疼痛评分、后伸、前屈、外展、内收、内旋、外旋角度等均高于对照组(均P<0.05),治疗组治疗后CGI-S评分低于对照组(P<0.05)。结论伤科黄水外敷可以缓解急性期肩周炎疼痛,改善肩关节功能障碍,促进康复。

Objective:To explore the effects of external application of Shangke Huangshui Gauze on acute scapulohumeral periarthritis.Methods:A total of 124 patients were divided into the control group and treatment group according to the random number table method,62 cases in each group.The control group was given conventional treatment,and the treatment group was given conventional treatment combined with external application of Shangke Huangshui Gauze.The clinical efficacy of the two groups,visual analog scale score(VAS)before and after treatment,Constant Murley shoulder joint function score(CMS),shoulder joint range of motion were analyzed and compared;the overall impression improvement score(CGI-S)after treatment,time to relieve pain and movement disorders were recorded.Results:The total effective rate in the treatment group was 96.77%,which was higher than 87.10%in the control group(P<0.05).The VAS scores of the two groups after treatment were lower than those before treatment,and the VAS score of the treatment group was lower than that of the control group after treatment(all P<0.05).After treatment,the patients’daily activity ability,shoulder joint range of motion,muscle strength,pain score,posterior extension,flexion,abduction,adduction,internal rotation,and external rotation angles were all higher than before treatment,and the daily activity ability,shoulder joint range of motion,muscle strength,pain score,posterior extension,flexion,abduction,adduction,internal rotation,and external rotation angles of the treatment group were all higher than those of the control group after treatment(all P<0.05)The CGI-S score of the treatment group was lower than the control group after treatment(P<0.05).Conclusion:External application of Shangke Huangshui Gauze can relieve the pain of acute scapulohumeral periarthritis,improve shoulder joint dysfunction,and promote rehabilitation.
