基于新产程的知信行干预联合拉玛泽减痛分娩法在初产妇中的应用

Application of Knowledge,Attitude and Practice Intervention Co Based on New Stage of Labor Combined with Lamaze Pain relief Delivery Method in Primipara

目的:探讨基于新产程的知信行干预联合拉玛泽减痛分娩法在初产妇中的应用效果。方法:选取2019年5月1日~2020年5月1日产科收治的初产妇164例,将其随机分为对照组和研究组各82例,对照组实施常规护理干预,研究组实施基于新产程的知信行干预联合拉玛泽减痛分娩法。比较两组产妇对相关知识的认知情况、不同产程时间、总产程时间、分娩疼痛程度、自然分娩后新生儿Apgar评分情况。结果:研究组对分娩过程、应对分娩不适技巧、影响分娩因素、母乳喂养、新生儿护理等知识认知情况优于对照组(P<0.05);研究组第一产程、第二产程、第三产程时间及总产程时间均短于对照组(P<0.01);研究组0~Ⅰ级疼痛和经阴道分娩新生儿Apgar评分中8~10分占比高于对照组(P<0.01,P<0.05)。结论:在初产妇中应用基于新产程的知信行干预联合拉玛泽减痛分娩法取得较好的干预效果,有效改善产妇分娩疼痛,缩短产妇经阴道分娩时间,提高其对分娩知识和新生儿护理知识的认识,促进产妇在分娩过程中意识到自身是一个积极角色,进而在分娩过程中充分配合,保证母婴在分娩过程中的安全,值得临床广泛推广。

Objective:To explore the application effect of knowledge,attitude and practice intervention based on new stage of labor combined with Lamaze analgesia delivery method for primipara.Methods:A total of 164 primipara admitted during May 1,2019 to May 1,2020 were selected and divided into the control group and the study group,82 cases in each group.In the control group,routine nursing intervention was given,and the study group implemented the new labor process based on the intervention of knowledge intervention combined with Ramazer pain reduction delivery method.Comparison was made between the two groups of women′s cognition on relevant knowledge,different labor duration,total labor consumption time,labor pain degree,neonatal Apgar score after natural delivery.Results:The knowledge on delivery process,skills for dealing with labor discomfort,factors influencing the childbirth,breastfeeding,and neonatal care was better in the study group than that in the control group(P<0.05);The first,second and third stages of labor,and the total duration in the study group were shorter than those in the control group(P<0.01).The proportion of 8 to 10 points in the Apgar scores of 0 to 1 grade vaginal pain and Apgar in the study group was higher in the study group than that in the control group(P<0.05).Conclusion:The application of new labor process-based intervention combined with Ramazer pain-reducing delivery method in primipara has achieved better intervention effect,can effectively improve maternal labor pain,shorten the time consumed by vaginal delivery,improve their understanding on delivery knowledge and neonatal nursing knowledge,and promote maternal awareness on its positive role in the process of delivery,and then give full play and cooperation in the process of delivery,so as to ensure the safety of mother and child in the process of delivery,which is worthy of clinical extensive promotion.
