经脐单孔腹腔镜后腹膜入路淋巴结切除三例

Three cases of peritoneal approach lymph node resection through transumbilical laparoendo⁃scopic single site surgery

目的探讨经脐单孔腹腔镜腹膜后淋巴结切除在妇科恶性肿瘤的可行性及应用前景。方法选取2020年1月-2月期间就诊于珠海市中西医结合医院的1例早期宫颈癌和2例早期子宫内膜癌,开展经脐单孔腹腔镜腹膜后淋巴结切除。观察淋巴结切除情况、手术时间、术后并发症。结果3例均成功完成了经脐单孔腹腔镜腹膜后淋巴结切除,平均切除淋巴结约70.66个,平均手术时间为121.3 min,术后均无并发症发生。结论经脐单孔腹腔镜腹膜后入路淋巴结切除对于妇科恶性肿瘤是切实可行的,且此术式经术者体位转换,无论是在地理位置还是术中操作便携性上都具有优势。

Objective To explore the feasibility and application prospect of peritoneal approach lymph node resection through transumbilical laparoendoscopic single site surgery(TU-LESS)in gynecological malignant tumors.Methods 1 case of early cervical cancer and 2 cases of early endometrial cancer who hosptialized in the Zhuhai Hospital of Integrated Traditional Chinese and Western Medicine from January to February 2020 were selected for peritoneal approach lymph node resection through TU-LESS.The lymph node resection,operation time,and postoperative complications of each case was observed.Results All 3 cases successfully completed transumbilical single port laparoscopic retroperitoneal lymph node resection,with an average of about 70.66 lymph nodes removed and an average operation time was 121.3 min,and there was no postoperative complications.Conclusions Transumbilical single site laparoscopic retroperitoneal lymph node resection is feasible surgical method for gynecological malignancies which has advantages in terms of geographic location and intraoperative portability.
