杂合型血液净化治疗维持性血液透析合并难治性高血压的临床效果观察

Observation of the clinical effect of heterozygous blood purification in the treatment of maintenance hemodialysis complicated with refractory hypertension

目的研究杂合型血液净化对维持性血液透析合并难治性高血压患者血液透析效果与血压水平的影响。方法选取2019年8月至2020年2月深圳市宝安人民医院收治的60例维持性血液透析合并难治性高血压患者,按照随机数字表法分为对照组与观察组,各30例。对照组患者采取单纯血液透析治疗,观察组患者采取杂合型血液净化治疗,两组患者均治疗8周。比较两组患者治疗前与治疗8周后收缩压、舒张压;比较血浆肾素、血管紧张素及甲状旁腺激素水平;比较治疗后并发症发生情况。结果与治疗前比,治疗8周后两组患者收缩压、舒张压、血浆肾素、血管紧张素及甲状旁腺激素水平均降低,且观察组低于对照组;治疗后观察组患者并发症总发生率低于对照组(均P<0.05)。结论杂合型血液净化对维持性血液透析合并难治性高血压患者具有良好的血液透析效果,可有效改善其血压水平,并有效清除患者机体内大分子毒素,维持内环境稳定,且安全性较好。

Objective To study the influence of heterozygous blood purification on the hemodialysis effect and blood pressure of maintenance hemodialysis patients with refractory hypertension.Methods 60 cases maintenance hemodialysis patients with refractory hypertension who were admitted to Shenzhen Baoan People's Hospital from August 2019 to February 2020 were divide into the control group and the observation group according to the random number table method,with 30 cases in each group.Patients in the control group were treated with hemodialysis alone,and patients in the observation group were treated with heterozygous blood purification.Both groups were treated for 8 weeks.Compared the systolic blood pressure,diastolic blood pressure,plasma renin,angiotensin,and parathyroid hormone levels of patients before treatment and 8 weeks after treatment between the two groups,and the occurrence of complications after treatment of patients in the two groups were compared.Results Compared with before treatment,after 8 weeks of treatment,the systolic blood pressure,diastolic blood pressure,plasma renin,angiotensin,parathyroid hormone levels of patients in the two groups decreased significantly,and the observation group was lower than the control group;after treatment,the total incidence of complications in the observation group was lower than that in the control group(all P<0.05).Conclusion Heterozygous blood purification has a good hemodialysis effect on maintenance hemodialysis patients with refractory hypertension,which can effectively improve the blood pressure levels and remove macromolecular toxins from the patient's body,maintain a stable internal environment,and has good safety.
