摘要
目的:观察富血小板血浆(PRP)联合负压封闭引流(VSD)对于慢性难愈性创面的疗效。方法:回顾性分析郑州人民医院血管外科2018年10月至2020年5月收治的慢性难愈性创面患者共74例。采用PRP联合VSD治疗39例,采用VSD治疗35例。治疗14 d后比较两组创面愈合率、创面愈合时间、细菌培养转阴率等。结果:PRP联合VSD组创面愈合率为92.3%(36/39),愈合时间为(39.15±4.18)d;VSD组为创面愈合率74.3%(26/35),愈合时间为(46.29±3.42)d;PRP联合VSD组创面愈合率大于VSD组(P=0.036),创面愈合时间短于VSD组(P<0.001)。两组治疗后白细胞计数和血清C反应蛋白水平均较治疗前降低,但降低程度差异无统计学意义(P>0.05)。PRP联合VSD组细菌培养转阴率为12/15,高于VSD组的5/13(P=0.031)。结论:PRP联合VSD治疗慢性难愈性创面安全有效。
Aim:To investigate the efficacy of platelet-rich plasma(PRP)combined with vacuum sealing drainage(VSD)for chronic refractory wounds.Methods:The data of 74 patients with chronic refractory wounds admitted to Zhengzhou People’s Hospital from October 2018 to May 2020 were retrospectively analyzed.A total of 39 cases underwent PRP combined with VSD,and 35 cases underwent VSD.After 14 day treatment,the wound healing rate,wound healing time,and bacterial culture conversion rate were compared between the two groups.Results:The wound healing rate was 92.3%(36/39)and the healing time was(39.15±4.18)days in the PRP combined with VSD group,74.3%(26/35)and(46.29±3.42)days in the VSD group;compared with VSD group,the healing rate of PRP combined with VSD group was higher(P=0.036),and wound healing time was shorter(P<0.001).White blood cell count and serum C-reactive protein level in both groups were lower than those before treatment,but there were no significant differences between the two groups(P>0.05).The negative conversion rate was 12/15 in PRP combined with VSD group,which was higher than 5/13 in VSD group(P=0.031).Conclusion:PRP combined with VSD is safe and effective in the treatment of chronic refractory wounds.
