院外创伤生命支持(PHTLS®)课程在军队医院应用价值探讨

Study on the practical value of prehospital trauma life support (PHTLS®) courses in military hospitals

目的探讨院外创伤生命支持(prehospital trauma life support,PHTLS)课程在军队医院医护人员战创伤救治训练中的应用价值。方法选取2019年1—12月在解放军总医院第六医学中心参加PHTLS培训的医护人员为研究对象,分析培训前后考试成绩和心率、体温、收缩压3项生理指标变化,并采用问卷调查的方法分析培训前后问卷调查评分变化。结果参训人员78人,其中医生64人,占82.1%,护理人员14人,占17.9%,5年以上临床工作经验参训人员占70.5%,经理论和模拟病例考核有72人通过,通过率92%。培训考核结束后体温、心率和收缩压3项生理指标均不同程度的升高(P<0.01),问卷调查培训前平均评分(50.1±3.468)分,培训后及培训后半年平均评分为(66.2±2.46)、(64.2±1.835)分,均高于培训前(P<0.01)。结论PHTLS培训能增加医护人员创伤救治知识,提高救护技术水平,增强信心,增加团队配合经验,对于军队医院医护人员战创伤救治训练有一定价值。

Objective To investigate the practical value of Prehospital Trauma Life Support(PHTLS)course in the train⁃ing of battlefield trauma care for medical staff in military hospitals.Methods The medical staff who participated in PHTLS training in the Sixth Medical Center of PLA General Hospital from January to December 2019 were selected.The examination results,heart rate,body temperature and systolic blood pressure before and after the training of the staff were analyzed.The questionnaire survey method was used to analyze the changes in questionnaire scores before and after the training.Results In 2019,three training sessions were held with 78 participants,including 64 doctors(82.1%),14 nurses(17.9%),55 trainees(70.5%)with more than five years'clinical work experience,and 72 trainees(92%)passed the examination.The body temperature,heart rate and systolic blood pressure were measured immediate⁃ly after the training,and these three physiological indexes were increased in varying degrees(P<0.01)compared with before the training.The questionnaire survey showed that the average score before training was(50.1±3.468)and that after training and half a year after training was(66.2±2.46)and(64.2±1.835)respectively,both higher than that before training(P<0.01).Conclusion The PHTLS training can increase medical staff's trauma care knowledge,improve res⁃cue technology level,enhance confidence,and increase team cooperation experience,and has a certain value for the training of medical staff in military hospitals.
