自拟祛斑汤结合烟酰胺凝胶治疗黄褐斑40例疗效观察

Clinical Efficacy of Self-made Quban Decoction Combined with Nicotinamide Gel in Treating 40 Cases of Melasma

目的:研究自拟祛斑汤结合烟酰胺凝胶对黄褐斑患者的临床疗效。方法:选取2017年7月至2019年6月在景德镇市皮肤病医院皮肤科诊治的黄褐斑患者80例,随机分为两组,即治疗组、对照组各40例,治疗组采用口服自拟祛斑汤+烟酰胺凝胶治疗,对照组采用口服氨甲环酸片+烟酰胺凝胶治疗,疗程为12周。观察治疗前后两组患者的皮损面积、皮损颜色以及计算皮损总积分的变化,对比两组患者的临床疗效。结果:经治疗后,治疗组总有效率为92.5%(37/40),对照组为75.0%(30/40),治疗组的临床疗效显著高于对照组(P<0.05)。与治疗前相比,两组患者皮损面积评分、皮损颜色评分、皮损总积分均显著下降,且治疗组比对照组下降更显著(P<0.05)。两组患者在疗程内均未发生明显的不良反应。结论:临床上采用自拟祛斑汤结合烟酰胺凝胶治疗黄褐斑,临床疗效确切,值得推广运用。

Objective:To study the clinical efficacy of self-made Quban decoction combined with nicotinamide gel on patients with chloasma.Methods:80 patients with chloasma treated in the dermatology department of Jingdezhen Dermatology Hospital from July 2017 to June 2019 were were randomly divided into 2 groups,namely the treatment group and the control group with 40 cases each.The treatment group was treated with oral self-made freckle decoction+nicotinamide gel,and the control group was treated with oral tranexamic acid tablet+niacinamide gel treatment,the course of treatment is 12 weeks.Observe the changes in the area,color of the skin lesions and the total score of the skin lesions of the two groups before and after treatment,and compare the clinical efficacy of the two groups.Results:After treatment,the total effective rate of the treatment group was 92.5%(37/40),and the control group was 75.0%(30/40).The clinical efficacy of the treatment group was significantly higher than that of the control group(P<0.05).Compared with before treatment,the skin lesion area score,skin color score,and total skin score of the two groups of patients decreased significantly,and the decrease in the treatment group was more significant than that of the control group(P<0.05).There were no obvious adverse reactions in the two groups of patients during the course of treatment.Conclusion:The self-made Quban decoction combined with nicotinamide gel in the treatment of chloasma is clinically effective and worthy of popularization.
