蠲痹饮治疗类风湿性关节炎的临床疗效及其对RF、CCP、ESR、CRP的影响

Clinical efficacy of Juanbi Decoction in rheumatoid arthritis and its influence on RF,CCP,ESR and CRP

目的探讨蠲痹饮治疗类风湿性关节炎的临床疗效及其对RF、抗CCP抗体、ESR、CRP的影响。方法选择2016年10月至2018年10月江苏省中医院门诊收治的70例类风湿性关节炎患者,按照开放、平行、对照的原则,分为雷公藤治疗组(n=35)和甲氨蝶呤对照组(n=35),雷公藤治疗组患者接受蠲痹饮治疗,甲氨蝶呤对照组患者接受甲氨蝶呤治疗,统计分析两组患者的主要症状、体征评分、实验室指标、临床疗效。结果雷公藤治疗组患者治疗后较治疗前的关节疼痛度评分降低幅度高于甲氨蝶呤对照组,差异有统计学意义(P<0.05),关节压痛数、关节肿胀数减少幅度均高于甲氨蝶呤对照组,差异有统计学意义(P<0.05),晨僵时间缩短幅度高于甲氨蝶呤对照组,差异有统计学意义(P<0.05)。雷公藤治疗组患者治疗后较治疗前的RF、抗CCP抗体、ESR、CRP水平降低幅度均高于甲氨蝶呤对照组,差异有统计学意义(P<0.05)。雷公藤治疗组患者治疗的总有效率为94.29%(33/35),高于甲氨蝶呤对照组的71.43%(25/35),差异有统计学意义(P<0.05)。结论类风湿性关节炎蠲痹饮治疗的临床疗效较甲氨蝶呤治疗显著,更能有效降低患者的RF、抗CCP抗体、ESR、CRP水平。

Objective To explore the clinical efficacy of Juanbi Decoction in rheumatoid arthritis(RA)and its influence on RF,anti-CCP antibody,ESR and CRP.Methods A total of 70 RA patients admitted to the outpatient department of Jiangsu Province Hospital of Chinese Medicine from October 2016 to October 2018 were selected as research objects.According to open,parallel and control principle,they were divided into the tripterygium wilfordii group(n=35)and the methotrexate group(n=35).Patients in the tripterygium wilfordii group received Juanbi Decoction,and patients in the methotrexate group received methotrexate.The main symptoms,signs scores,laboratory indicators,and clinical efficacy of the two groups were statistically analyzed.Results The decrease of joint pain score after treatment was significantly higher in the tripterygium wilfordii group than that in the methotrexate group,with statistically significant differences(P<0.05).The decreases of tender joint count and swollen joint count were significantly higher in the tripterygium wilfordii group than that in the methotrexate group,with statistically significant differences(P<0.05).The shortening of the duration of morning stiffness was significantly higher in the tripterygium wilfordii group than that in the methotrexate group,with statistically significant differences(P<0.05).After treatment,the decrease of the levels of RF,anti-CCP antibody,ESR and CRP were significantly higher in the tripterygium wilfordii group than those in the methotrexate group,with statistically significant differences(P<0.05).The total effective rate of the tripterygium wilfordii group was 94.29%(33/35),which was significantly higher than that of(71.43%[25/35])in the methotrexate group,with statistically significant differences(P<0.05).Conclusion The clinical efficacy of Juanbi Decoction in rheumatoid arthritis is better than that of methotrexate,and it can effectively reduce the levels of RF,anti-CCP antibody,ESR and CRP.
