二甲双胍结合克罗米芬治疗多囊卵巢综合征并不孕症疗效观察

Efficacy of metformin combined with clomiphene in the treatment of polycystic ovary syndrome with infertility

目的分析二甲双胍结合克罗米芬联合用药在多囊卵巢综合征合并不孕症患者治疗中的应用疗效。方法筛选2016年12月~2017年12月莒县中医医院收治的多囊卵巢综合征并不孕症患者100例,采用随机数表法将患者分为对照组和实验组,每组50例。单纯使用克罗米芬药物治疗的患者作为对照组,在对照组基础上服用二甲双胍药物的患者作为实验组,分析两组间患者的排卵、妊娠、早孕流产情况以及体内激素情况。对比两组患者月经稀发、痤疮、多毛等临床症状。对比两组促黄体生成素(LH)、雌二醇(E2)、睾酮(T)。结果实验组在治疗过后排卵率、妊娠率比对照组高,早孕流产率比对照组低,差异显著(P<0.05);另外,实验组月经稀发、痤疮、多毛症状少于对照组,差异显著(P<0.05),实验组LH、T低于对照组,差异有统计学意义(P<0.05),而E2两组对比差异无统计学意义(P>0.05)。结论二甲双胍结合克罗米芬能够帮助多囊卵巢综合征并不孕症患者改善机体内分泌,促进患者排卵,提高患者妊娠概率,降低患者早孕流产率。

Objective To analyze the efficacy of metformin combined with clomiphene in the treatment of patients with polycystic ovary syndrome complicated with infertility.Methods A total of 100 patients with polycystic ovary syndrome and infertility who were treated in Juxian Hospital of Traditional Chinese Medicine from December 2016 to December 2017 were screened.The patients were divided into control group and observation group by random number table method,with 50 cases in each group.The patients treated with clomiphene alone were used as the control group,and the patients treated with metformin on the basis of the control group were used as the experimental group.Ovulation,pregnancy,early pregnancy abortion and the situation of hormones in the patients between the two groups were analyzed.The clinical symptoms of menstrual thinness,acne and hirsutness were compared between the two groups.Luteinizing hormone(LH),estradiol(E2)and testosterone(T)were compared between two groups.Results After treatment,the ovulation rate and pregnancy rate of the experimental group were higher than those of the control group,and the abortion rate of early pregnancy was lower than that of the control group,the difference was significant(P<0.05).In addition,the symptoms of hypomenorrhea,acne and hairy symptoms in the experimental group were less than those in the control group,the difference was significant(P<0.05),LH and T in the experimental group were lower than those in the control group,the difference was statistically significant(P<0.05),while there was no significant difference in E2 between the two groups(P>0.05).Conclusion Metformin combined with clomiphene can improve endocrine secretion,promote ovulation,improve pregnancy probability and reduce early pregnancy abortion in patients with polycystic ovary syndrome and infertility.
