老年前列腺增生患者经尿道前列腺汽化电切术后膀胱颈挛缩的影响因素

Influencing Factors of Bladder Neck Contracture After Transurethral Vaporization of Prostate in Elderly Patients with Benign Prostatic Hyperplasia

目的探讨老年前列腺增生(BPH)患者经尿道前列腺汽化电切术(TUVP)后膀胱颈挛缩(BNC)的影响因素。方法选取2019年10月至2020年10月在罗山县人民医院行TUVP后发生BNC的60例老年BPH患者作为发生组,另收集同期在医院行TUVP后未发生BNC的60例老年BPH患者作为未发生组。查阅并收集患者基本资料,经单因素与多因素分析行TUVP的老年BPH患者术后BNC发生的影响因素。结果两组前列腺体积、合并前列腺炎、膀胱冲洗温度、膀胱冲洗液速度、导尿管气囊注水量比较,差异有统计学意义(P<0.05);两组其他基线资料比较,差异无统计学意义(P>0.05)。经单因素分析后,建立多因素回归模型分析显示,BPH体积较小、合并前列腺炎、膀胱冲洗温度较低、膀胱冲洗液速度较慢及导尿管气囊注水量较多是老年BPH患者TUVP后BNC的影响因素(OR>1,P<0.05)。结论BPH体积较小、合并前列腺炎、膀胱冲洗温度较低、膀胱冲洗液速度较慢及导尿管气囊注水量较多是老年BPH患者TUVP后BNC的影响因素。

Objective To investigate the influencing factors of bladder neck contracture(BNC)in elderly patients with benign prostatic hyperplasia(BPH)after transurethral vaporization of prostate(TUVP).Methods A retrospective analysis was conducted,and the clinical data of 60 elderly patients with BPH who developed BNC after TUVP in Luoshan County People’s Hospital from October 2019 to October 2020 were collected and set as occurrence group.In addition,the clinical data of 60 elderly patients with BPH who did not develop BNC after TUVP in the hospital during the same period were collected and set as non-occurrence group.The basic information of patients was consulted and collected.Univariate analysis and multivariate analysis were used to analyze the influencing factors of BNC in elderly patients with BPH after TUVP.Results There were differences in prostate volume,combined prostatitis,temperature of bladder irrigation,speed of bladder irrigation fluid and water injection volume of catheter balloon between the two groups(P<0.05).There was no difference in other baseline data between the two groups(P>0.05).After single factor analysis,the multiple regression model was established,and the result showed that smaller volume of BPH,combined prostatitis,lower temperature of bladder irrigation,slower speed of bladder irrigation fluid and more amount of water injection of catheter balloon were the influencing factors of BNC after TUVP in elderly patients with BPH(OR>1,P<0.05).Conclusion Small volume of BPH,combined prostatitis,lower temperature of bladder irrigation,slower speed of bladder irrigation fluid and more amount of water injection of catheter balloon are the influencing factors of BNC after TUVP in elderly patients with BPH.
