三维适形放疗联合PPF化疗方案治疗中晚期食管癌的效果

Effect of Three Dimensional Conformal Radiotherapy Combined with PPF Chemotherapy Regimen in the Treatment of Middle and Advanced Esophageal Cancer

目的探讨三维适形放疗(3D-CRT)联合PPF(顺铂、平阳霉素、5-氟尿嘧啶)化疗方案治疗中晚期食管癌(EC)的效果。方法选取2017年5月至2020年5月南阳市中心医院收治的90例中晚期EC患者,按照随机数表法分为对照组(45例)与观察组(45例)。对照组接受3D-CRT治疗,观察组接受3D-CRT联合PPF化疗方案治疗,均连续治疗3个周期。治疗3个周期后,比较两组近期疗效、免疫功能及治疗期间药物毒副作用。结果治疗3个周期后,两组CD3^(+)/CD4^(+)水平均低于治疗前,CD3^(+)/CD8^(+)水平高于治疗前,差异有统计学意义(P<0.05);两组间CD3^(+)/CD4^(+)、CD3^(+)/CD8^(+)、CD4^(+)/CD8^(+)水平比较,差异无统计学意义(P>0.05);治疗期间,观察组与对照组药物毒副作用发生率比较,差异无统计学意义(P>0.05)。结论采用3D-CRT联合PPF化疗方案治疗中晚期EC可提高临床效果,且不增加单纯放疗引起的免疫抑制状态及药物毒副作用。

Objective To investigate the effect of three-dimensional conformal radiotherapy(3D-CRT)combined with PPF(cisplatin,bleomycin,5-fluorouracil)chemotherapy regimen in the treatment of middle and advanced esophageal cancer(EC).Methods Ninety patients with middle and advanced EC treated in Nanyang Central Hospital from May 2017 to May 2020 were selected,they were divided into control group(45 cases)and observation group(45 cases)according to the random number table method.The control group was treated with 3D-CRT,and the observation group was treated with 3D-CRT combined with PPF chemotherapy regimen,all patients were treated for 3 consecutive cycles.After 3 cycles of treatment,the short-term efficacy,immune function and toxic and side effect of drugs during treatment were compared between the two groups.Results After 3 cycles of treatment,the levels of CD3^(+)/CD4^(+)in both groups were lower than those before treatment,and the levels of CD3^(+)/CD8^(+)were higher than those before treatment(P<0.05).However,there were no differences in the levels of CD3^(+)/CD4^(+),CD3^(+)/CD8^(+),CD4^(+)/CD8^(+)between the two groups(P>0.05).During treatment,there was no difference in the incidence of toxic and side effect in the observation group and the control group(P>0.05).Conclusion 3D-CRT combined with PPF chemotherapy regimen can improve the clinical efficacy in the treatment of EC,and it will not increase the immunosuppressive state and toxic and side effect caused by radiotherapy alone.
