维生素D联合屈螺酮炔雌醇治疗多囊卵巢综合征患者的效果

Efficacy of Vitamin D Combined with Drospirenone and Ethinyl Estradiol in the Treatment of Patients with Polycystic Ovary Syndrome

目的探讨维生素D联合屈螺酮炔雌醇治疗多囊卵巢综合征(PCOS)的效果。方法选取2019年1月至2020年8月郑州颐和医院收治的112例PCOS患者作为研究对象,按随机数表法分成联合组与常规组,各56例。常规组接受屈螺酮炔雌醇治疗,联合组接受维生素D联合屈螺酮炔雌醇治疗。比较两组总有效率,治疗前、治疗6个月性激素指标[促卵泡激素(FSH)、黄体生成素(LH)、雌二醇(E_( 2))、睾酮(T)]水平、脂代谢指标[低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)]、氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)]、miRNA-93、miRNA-141相对表达量。结果联合组总有效率[92.86%(52/56)]高于常规组[76.79%(43/56)],差异有统计学意义(P<0.05)。治疗6个月,联合组血清LH、T水平低于常规组,血清 E_( 2) 、FSH水平高于常规组,差异有统计学意义(P<0.05)。治疗6个月,联合组TG、LDL-C、TC水平低于常规组,HDL-C水平高于常规组,差异有统计学意义(P<0.05)。治疗6个月,联合组血清MDA水平低于常规组,SOD水平高于常规组,差异有统计学意义(P<0.05)。治疗6个月,联合组miRNA-93、miRNA-141相对表达量低于常规组,差异有统计学意义(P<0.05)。结论维生素D联合屈螺酮炔雌醇治疗PCOS患者效果确切,可有效调节性激素及miRNA-93、miRNA-141基因相对表达量,改善脂代谢水平,缓解氧化应激。

Objective To explore the therapeutic effect of vitamin D combined with drospirenone and ethinyl estradiol in the treatment of patients with polycystic ovary syndrome(PCOS).Methods A total of 112 patients with PCOS in Zhengzhou Yihe Hospital from January 2019 to August 2020 were selected as the research subjects.The patients were divided into combined groups(56 cases)and conventional groups(56 cases)according to the random number table method.The conventional group received drospirenone and ethinyl estradiol treatment,and the combination group received vitamin D combined with drospirenone and ethinyl estradiol treatment.The total effective rate of the two groups was compared.The levels of sex hormones[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_( 2)),testosterone(T)],lipid metabolism index[low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),triglycerides(TG)],oxidative stress index[malondialdehyde(MDA),superoxide dismutase(SOD)],the relative expression of miRNA-93,miRNA-141 were compared between the two groups before treatment and 6 months after treatment.Results The total effective rate of the combined group[92.86%(52/56)]was higher than that of the conventional group[76.79%(43/56)](P<0.05).The serum LH and T levels of the combined group were lower than those of the conventional group after 6 months of treatment.Serum E_( 2) and FSH levels were higher than those in the conventional group(P<0.05).The TG,LDL-C,and TC levels in the combined group of 3 months and 6 months after treatment were lower than those in the conventional group,and the levels of HDL-C were higher than those in the conventional group(P<0.05).The serum MDA level of the combined group was lower than that of the conventional group 6 months after treatment,and the SOD level was higher than that of the conventional group(P<0.05).The relatively expression level of miRNA-93 and miRNA-141 of the combined group were lower than that of the conventional group 6 months after treatment(P<0.05).Conclusion Vitamin D combined with drospirenone and ethinyl estradiol has a significant effect in the treatment of PCOS patients.It can effectively regulate the relative expression of sex hormones and miRNA-93 and miRNA-141 genes,improve lipid metabolism,and relieve oxidative stress.
