抑郁症患者抑郁水平、心理困扰及述情障碍的关系研究

Research on the relationship between depression level and psychological distress in patients with depression

目的探讨抑郁症患者抑郁水平和心理困扰的关系,以及述情障碍在其中的中介作用。方法以我院精神科2019年5月-2020年5月收治的136例抑郁症患者为研究对象。采用贝克抑郁症量表(BDI)、简易凯斯勒心理困扰状况评定量表(K10)以及多伦多述情障碍量表(TAS)对两组受试者的抑郁水平、心理困扰及述情障碍进行评估,采用Pearson相关因素进行分析患者抑郁水平和心理困扰之间的关系,采用Amos 22.0软件建立述情障碍的中介效应模型。结果本组患者的BDI评分为(26.39±3.64)分,K10量表总分及条目均分分别为(35.38±4.29)分和(3.33±1.28)分,TAS量表评分为(73.49±6.39)分;抑郁水平和心理困扰、述情障碍呈正相关(r=0.355,0.346,P=0.000);心理困扰和述情障碍可以直接预测抑郁症患者的抑郁水平(直接路径系数分别为0.084,0.116),心理困扰也可以通过述情障碍间接预测其抑郁水平(间接路径系数为0.0311)。结论抑郁症患者的抑郁水平与心理困扰有密切关系,述情障碍在两者之间发挥部分中介作用,应重点关注患者的心理情况和述情能力,以改善抑郁症患者的抑郁症状。

Objective To explore the relationship between depression level and psychological distress in patients with depression,and the mediating role of alexithymia in depression.Methods 136 patients with depression admitted to the psychiatric department of our hospital from May 2019 to May 2020 were taken as the observation group.Using the Beck depression inventory(BDI),simple Kessler psychological distress status rating scale(K10)and Toronto alexithymia scale(TAS)to evaluate the levels of depression,psychological distress,and alexithymia of the patients.Using Pearson correlation analysis to analyze the relationship between the depression level and psychological distress in patients.Amos 22.0 software was adopted to establish the mediation effect of alexithymia model.Results BDI scores of patients was(26.39±3.64)scores,The total score and mean score of items in K10 scale were(35.38±4.29)and(3.33±1.28)sores,and TAS scale score was(73.49±6.39)sores.Psychological distress and alexithymia can directly predict the level of depression in patients with depression(direct pathway coefficient is 0.084,0.116,respectively),and psychological distress can also indirectly predict the level of depression through alexithymia(indirect pathway coefficient is 0.0311).Conclusion There is a close relationship between the level of depression and psychological distress in patients with depression,and alexithymia plays a part of the mediating role between those two sides.We should pay more attention to the patients'psychological condition and alexithymia ability to improve the symptoms of depression in patients with depression.
