肝素结合蛋白及IL-17 IL-10在慢性阻塞性肺疾病急性加重期的表达与意义 被引量:4

摘要 目的探讨肝素结合蛋白(HBP)及IL-17、IL-10在慢性阻塞性肺疾病(COPD)急性加重期的表达及临床意义。方法选取2019年1月至2019年12月于本院就诊的COPD患者235例,其中COPD稳定期患者85例,COPD急性加重期(AEAOPD)患者150例,另选取同期于本院健康体检者50例,并根据痰培养结果将COPD急性加重期(AECOPD)患者分为细菌感染组(82例)和非细菌感染组(68例)。比较各组间HBP及IL-17、IL-10水平的差异,比较AECOPD细菌感染组与非细菌感染组HBP、IL-17、IL-10及血氧分压(PO_(2))、二氧化碳分压(PCO_(2))、PH值的差异,分析AECOPD组血PCh、PCCh、PH值与HEP、IL-17、IL-10的相关性。结果COPD稳定组、AECOPD组的HBP及IL-17水平显著高于对照组,IL-10水平显著低于对照组,AECOPD组的HBP及IL-17水平显著高于COPD稳定组,IL-10水平显著低于COPD稳定组。AECOPD细菌感染组的HBP及IL-17水平显著高于非细菌感染组,IL-10水平显著低于非细菌感染组,血PO_(2)、PCO_(2)、PH值在AECOPD细菌感染组与非细菌感染组间无显著性差异(P>0.05)。COPD急性加重期血PCO_(2)与HBP、IL-17呈正相关,与IL-10呈负相关,PH值与HBP、IL-17呈负相关,与IL-10呈正相关。结论HBP、IL-17、IL-10及血气分析相关指标与COPD病情急性加重密切相关,可以用于评估病情的严重程度和指导临床诊疗. Objective To explore the expression and clinical significance of heparin binding protein(HBP),IL-17 and IL-10 in the acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods A total of 235 COPD patients who were treated in our hospital from January 2019 to December 2019 were selected,including 85 patients with stable COPD,150 patients in acute exacerbation of COPD(AEAOPD),and 50 patients who received physical examination in our hospital during the same period.According to the results of sputum culture,patients with acute exacerbation of COPD(AECOPD)were divided into bacterial infection group(82 cases)and non-bacterial infection group(68 cases).Compare the difierences in the levels of HBP,IL-17,and IL-10 between the groups,compare HBP,IL-17,IL-10,partial pressure of blood oxygen(PO_(2)),and partial pressure of carbon dioxide(PCO_(2))between the AECOPD bacterial infection group and the non-bacterial infection group,PH value difierence,analyze the correlation between blood PO_(2),PCO_(2),PH value and HBP,IL-17,IL-10 in the AECOPD group.Results The levels of HBP and IL-17 in the COPD stable group and the AECOPD group were significandy higher than the control group,and the IL-10 level was significantly lower than the control group.The HBP and IL-17 levels in the AECOPD group were significandy higher than the COPD stable group,and the IL-10 level was significant lower than the COPD stable group.The levels of HBP and IL-17 in the AECOPD bacterial infection group were significantly higher than those in the non-bacterial infection group,and the IL-10 levels were significantly lower than those in the non-bacterial infection group.The blood PO_(2),PCO_(2),and PH values were not significant diflerence(P>0.05)between the AECOPD bacterial infection group and the non-bacterial infection group.During the acute exacerbation of COPD,blood PCO_(2)was positively correlated with HBP and IL-17,and negatively correlated with IL-10.PH value was negatively correlated with HBP and IL-17,and positively correlated with IL-10.Conclusion HBP,IL-17,IL-10 and blood gas analysis related indicators are closely related to the acute exacerbation of COPD,and can be used to assess the severity of the disease and guide clinical diagnosis and treatment.
