BSSRO后退下颌骨术后的下颌角形态对比研究 被引量:1

摘要 目的通过对下颌前突患者在接受双侧下颌支矢状劈开截骨术(bilateral sagittal split ramus osteotomy,BSSRO)前后下颌角形态变化的分析,研究BSSRO对患者面形轮廓的影响。方法回顾分析自2018年6月至2020年6月于中国医科大学附属口腔医院口腔颌面外科接受BSSRO手术诊疗的骨性Ⅲ类错畸形35例患者的临床资料,术前1周(T1)及术后8~12个月(T2)分别进行三维CT的拍摄,并使用Adobe Photoshop CS6与MaterialiseProPlan CMF 3.0进行图像处理和测量研究。结果BSSRO术后下颌前突患者的下颌骨宽度较术前增加约2.47 mm,且其术前与术后的t检验结果差异具有统计学意义(P<0.01);全面高有所减小,下颌角角度整体减小,下颌平面角与下颌支坡度(下颌支平面与眶耳平面夹角)增大。结论对于骨性Ⅲ类错畸形的患者,BSSRO术后面下1/3宽度存在增大趋势。 Objective To analyze the changes of morphology of mandibular angle after bilateral sagittal split ramus osteotomy(BSSRO).To study the effect of BSSRO on facial contour of patients.Methods The clinical data of 35 patients with skeletal classⅢmalocclusion from June 2018 to June 2020 were retrospectively analyzed.Three-dimensional CT was taken at 1 week before operation(T1)and 8 to 12 months after operation(T2).The Adobe Photoshop CS6 and MaterialiseProPlan CMF 3.0 were used for image processing and measurement.Results Compared with mandibular width before operation,the mandibular width increased 2.47 mm after BSSRO.The paired t-test showed that the difference before and after operation was statistical significant(P<0.01).The mandibular angle decreased,the mandibular plane angle and the angle between mandibular branch plane and Frankfurt horizontal plane(F-H plane)increased.Conclusion For patients with skeletal ClassⅢmalocclusion,the width of mandibular angle after BSSRO tends to increase.
