整形缝合技术在急诊面部浅表外伤手术中的应用:720例回顾性分析

Application of plastic suture technique in the emergency surgical treatment of superficial facial trauma:retrospective study of 720 patients

目的探讨整形缝合技术在急诊面部浅表外伤手术中的应用效果。方法回顾性分析自2012年1月至2018年1月,中国医学科学院北京协和医学院整形外科医院整形十六科由同一术者采用整形缝合技术治疗面部浅表外伤患者的临床资料,采集患者术前、术后7 d、术后6个月的照片,统计患者基线资料、创伤类型、伤口位置、伤口长度及深度、手术细节、并发症、瘢痕外观和患者满意度。结果共纳入720例患者,年龄2~76岁,平均年龄为(17.89±13.40)岁,平均随访时间为(8.15±3.21)个月(6~11个月)。创伤类型为撕裂伤(675例,93.75%)、挫伤(18例,2.50%)、动物咬伤(9例,1.25%)、人咬伤(3例,0.42%)、其他(15例,2.08%)。最常见的损伤部位为额部(20%)、眉及上睑部(20%)、下颌及颏部(20%),其次为唇部(15%)、面颊部(10%)、外鼻(5%)、颞部(5%)、其他(5%)。伤口平均长度(3.25±2.17)cm,在额部和颏部深达骨质表面,其他部位深及皮下层。术后并发症发生率为0.30%(感染2例)。大部分患者术后瘢痕不明显,效果较满意。结论采用整形缝合技术治疗急诊面部外伤可获得良好的功能和美观效果。术后并发症较少,瘢痕不明显,患者满意度较高。

Objective To explore the efficacy of plastic suture technique in the emergency surgical treatment of superficial facial trauma.Methods The clinical data of patients with superficial facial trauma underwent plastic suture technique treatment from January 2012 to January 2018 were retrospectively reviewed.Preoperative,seventh-day and sixth-month postoperative photographs were taken.The baseline information,type of trauma,location,length and depth of wounds,details of operative procedures,complications,appearance of scars and patients'satisfaction were recorded.Results A total of 720 patients were included in this study.The age of patients ranged from 2 to 76-year-old,with an average of 17.89-year-old.The follow-up period ranged from 6 to 11 months,with an average of(8.15±3.21)months.The types of trauma were laceration(93.75%),contusion(2.50%),animal bite wounds(1.25%),human bite wounds(0.42%)and other wounds(2.08%).The forehead(20%),eyebrow and upper eyelid(20%),mandible and chin(20%)were the most common injury locations,followed by lip(15%),cheek(10%),nose(5%),temporal region(5%)and other parts(5%).The average length of wounds was(3.25±2.17)cm.The wounds of forehead and chin were deep to bony surface and other parts were deep to subcutaneous layer.The incidence rate of complications was 0.30%.The postoperative scar was not obvious in most patients.The efficacy was satisfactory.Conclusion The application of plastic suture technique in the treatment of emergency facial trauma could obtain good function and aesthetic effect.The postoperative complications are rare and the scars are not obvious.Patients'satisfaction is high.
