乳腺癌患者血清中miR-27a表达水平及其临床意义

The serum levels and clinical significance of MiR-27a in patients with breast cancer

目的探讨乳腺癌患者血清中miR-27a表达水平及其临床意义。方法回顾性纳入2018年6月1日—2019年10月1日于本院就诊的95例乳腺癌患者(乳腺癌组)、35例乳腺良性增生患者(乳腺良性增生组)和30例健康体检者(对照组)作为研究对象。于清晨空腹抽取3组研究对象外周静脉血5 ml,采用RT-PCR法检测并比较血清中miR-27a表达水平的差异,同时分析乳腺癌患者血清中miR-27a表达水平与病理特征的关系和影响因素。利用ROC曲线分析血清中miR-27a表达水平在鉴别乳腺癌与乳腺良性增生及健康对照中的临床效能。结果乳腺癌组、乳腺良性增生组和对照组血清中miR-27a表达水平分别为0.61、0.48和0.18,与乳腺良性增生组和对照组相比,乳腺癌组血清中miR-27a表达水平显著升高,差异均有统计学意义(U=26.55,U=63.24,P<0.05);与对照组相比,乳腺良性增生组血清中miR-27a表达水平显著升高,差异有统计学意义(P<0.05)。肿瘤直径≥2 cm、存在淋巴结转移、分化程度为中~低分化和TNM分期为Ⅲ/Ⅳ期的乳腺癌患者血清中miR-27a表达水平显著高于肿瘤直径<2 cm、未发生淋巴结转移、高分化程度和TNM分期为Ⅰ/Ⅱ期的患者,差异均有统计学意义(P<0.05)。分化程度、淋巴结转移、TNM分期均是乳腺癌患者血清中miR-27a表达水平升高的危险因素。结论乳腺癌患者血清中miR-27a表达水平显著升高,与临床病理特征密切相关,有望成为诊断乳腺癌的生物学标志物之一。

Objective To investigate the serum levels of MiR-27a in patients with breast cancer and its clinical significance.Methods A total of 95 patients with breast cancer(breast cancer group),35 patients with breast hyperplasia(breast hyperplasia group)and 30 healthy individuals(control group)were enrolled in our hospital from June 1,2018 to October 1,2019.About 5 ml of peripheral venous blood was extracted from all subjects with an empty stomach in the early morning.The difference of serum miR-27a levels in all subjects were detected and compared by RT-PCR methods.RT-PCR was used to detect and compare the differences in the serum levels of miR-27a in breast cancer patients,and the relationship between the serum levels of miR-27a,the pathological characteristics and the influencing factors were analyzed.The clinical efficacy of miR-27a in the diagnosis of breast cancer from breast hyperplasia and healthy control were analyzed by the receiver operating characteristic(ROC)curve.Results The levels of miR-27a in breast cancer group,benign hyperplasia group and control group were 0.61,0.48 and 0.18,respectively.Compared with the benign hyperplasia group and the control group,the levels of miR-27a in the breast cancer group was significantly higher,and the differences were statistically significant(U=26.55,U=63.24,P<0.05).Compared with the control group,the levels of miR-27a in the benign hyperplasia group were significantly higher,and the difference was statistically significant(P<0.05).The serum levels of miR-27a observed in patients with tumor size more than 2 cm,lymph node metastasis,middle-poor differentiation and TNM stage III/IV were higher than that in patients with tumor size lower than 2 cm,without lymph node metastasis,high differentiation and TNM stage I/II,and the difference was statistically significant(P<0.05).The differentiation degree,lymph node metastasis and TNM stage were risk factors for the increased serum miR-27a levels in breast cancer patients.Conclusion The serum level of miR-27a in patients with breast cancer is significantly higher and it’s related to the clinical characteristics,which may be a potential biomarker for the diagnosis of breast cancer.
