低血红蛋白密度对于诊断缺铁性贫血的分析

Analysis of low hemoglobin density for the diagnosis of iron deficiency anemia

目的通过检验红细胞低血红蛋白密度(LHD),分析其对于诊断缺铁性贫血(IDA)的应用效果,为临床诊断和防治提供参考依据。方法回顾性分析本院2015年—2019年就诊的门诊及住院的贫血患者450例,分为缺铁性贫血(IDA)组140例、肾性贫血组140例、肿瘤性贫血组140例和健康体检组30例。分别使用血细胞分析仪去测定低血红蛋白密度(LHD)、红细胞(RBC)、平均红细胞容积(MCV)、平均红细胞血红蛋白(MCH)、平均红细胞血红蛋白浓度(MCHC),并对各项参数进行比较,分析LHD在各组中的诊断程度。结果缺铁性贫血组中LHD水平高于肾性贫血组、肿瘤性贫血组和健康体检组,差异均有统计学意义(P<0.05);LHD对IDA诊断的敏感度为99.8%、特异度为97.4%、AUC为0.98。结论LHD对于诊断IDA具有较高的敏感度和特异度,且LHD在仅IDA患者中才会有明显的升高,在其他不伴随缺铁性贫血的贫血类别中升高程度较低,即可作为临床诊断缺铁性贫血的参考依据。

Objective By examining the low hemoglobin density(LHD)of red blood cells and analyzing its application effect in the diagnosis of iron deficiency anemia(IDA),this paper provides a reference for clinical diagnosis and prevention.Methods A retrospective analysis of 450 outpatients and inpatients with anemia in our hospital from 2015 to 2019 was divided into 140 cases of iron deficiency anemia(IDA)group,140 cases of renal anemia group,140 cases of tumor anemia group and 30 cases of health examination group.The low hemoglobin density(LHD),red blood cell(RBC),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH)and mean corpuscular hemoglobin concentration(MCHC)were measured by hematology analyzer,and the degree of diagnosis of LHD in each group was analyzed.Results The level of LHD in the iron deficiency anemia group was higher than that in the renal anemia group,tumor anemia group and healthy physical examination group,with the differences statistically significant(P<0.05);the sensitivity of LHD to IDA diagnosis was 99.8%,specificity was 97.4%,and AUC was 0.98.Conclusion LHD has a high sensitivity and specificity for the diagnosis of IDA,and LHD is significantly increased in only IDA patients.In other anemia categories without iron deficiency anemia,LHD has a low degree of increase,which can be used as a reference for clinical diagnosis of iron deficiency anemia.
