摘要
目的探讨老年慢性心力衰竭(CHF)患者N端B型钠尿肽前体(NT-proBNP)与辅助性T细胞相关因子的表达水平及其临床价值。方法选取2019年1月—12月本院住院的慢性心力衰竭患者125例,根据美国纽约心脏协会(NYHA)心功能分级分Ⅱ~Ⅳ3组,同时选取40例健康体检者作为对照组。比较各组间各类辅助性T细胞水平的变化及NT-proBNP、IFN-γ、IL-17、IL-10水平的差异,同时比较Ⅱ~Ⅳ不同心功能分级分组间各类指标的变化。结果CHF组CD3、CD4、CD4/CD8水平明显低于对照组(P<0.05),CD8水平明显高于对照组(P<0.05)。NYHA分组患者CD3、CD4、CD4/CD8水平逐级下降(P<0.05),CD8水平逐级升高(P<0.05)。CHF组NT-proBNP、IFN-γ和IL-17水平显著高于对照组(P<0.05),IL-10水平显著低于对照组(P<0.01)。NYHA分组患者NT-proBNP、IFN-γ、IL-17、IL-10各指标比较差异均有统计学意义(P<0.01)。结论NT-proBNP联合辅助性T细胞相关因子检测可以评估CHF患者病情的严重程度,用于指导临床诊疗。
Objective To investigate the expression and clinical value of N-terminal pro-B-type natriuretic peptide(NT proBNP)and helper T cell related factors in elderly patients with chronic heart failure(CHF).Methods A total of 125 patients with chronic heart failure who were hospitalized in our hospital from January 2019 to December 2019 were selected and divided into three groups according to NYHA cardiac function classificationⅡ-Ⅳ,and 40 healthy people were selected as the control group.The levels of NT proBNP,IFN-γ,IL-17 and IL-10 were compared between CHF group and control group,and the changes of various indexes betweenⅡ-Ⅳgroups were compared.Results The levels of CD3,CD4 and CD4/CD8 in CHF group were significantly lower than those in the control group.The levels of CD3,CD4 and CD4/CD8 decreased and the levels of CD8 increased in NYHA patients(P<0.05).The levels of NT proBNP,IFN-γand IL-17 in CHF group were significantly higher than those in the control group,and the levels of IL-10 were significantly lower than those in the control group(P<0.01).The indexes of NT proBNP,IFN-γ,IL-17 and IL-10 in each NYHA group were statistically significant(P<0.01).Conclusion NT proBNP combined with helper T cell associated factors could be used to evaluate the severity of chronic heart failure in the elderly,which could also guide clinical diagnosis and treatment.
