血清闭锁蛋白、闭锁小带蛋白1表达与急性脑梗死患者认知障碍的相关性 被引量:9

Correlation between the levels of serum occluding, zonula occludens-1 and cognitive dysfunction in patients with acute cerebral infarction

摘要 目的探讨血清闭锁蛋白、闭锁小带蛋白1(ZO-1)水平与急性脑梗死患者认知障碍的关系。方法根据蒙特利尔认知评估量表(MoCA)评分将240例急性脑梗死患者分为认知功能障碍组(<26分)104例和认知功能正常组(≥26分)136例。采用ELISA检测血清闭锁蛋白、ZO-1水平。采用Pearson法分析血清闭锁蛋白、ZO-1水平与急性脑梗死患者并发认知功能障碍的相关性,采用二元Logistic回归分析筛选影响急性脑梗死患者认知功能障碍的相关危险因素。结果认知功能障碍组患者血清闭锁蛋白、ZO-1水平及MoCA评分显著低于认知功能正常组(均P<0.05)。急性脑梗死患者血清中闭锁蛋白、ZO-1水平与MoCA评分均呈正相关(r=0.614,P=0.000;r=0.607,P=0.000)。多因素Logistic回归分析显示,糖尿病为急性脑梗死患者认知功能障碍的危险因素(P=0.009),闭锁蛋白、ZO-1水平为急性脑梗死患者认知功能障碍的保护因素(P=0.007,P=0.013)。结论急性脑梗死并发认知功能障碍患者血清中闭锁蛋白、ZO-1水平呈低表达,可能是急性脑梗死并发认知功能障碍的保护因素,而糖尿病是危险因素。 Objective To investigate the correlation between the levels of serum occludin,zonula occludens-1(ZO-1)and cognitive dysfunction in patients with acute cerebral infarction.Methods A total of 240 patients with acute cerebral infarction were divided into the cognitive dysfunction group(<26 points,104 cases)and the normal cognitive function group(≥26 points,136 cases)according to the score of the Montreal cognitive assessment scale(MoCA).The levels of occludin and ZO-1 in serum were detected by ELISA.The correlation between the levels of occludin and ZO-1 in serum and the cognitive dysfunction in patients with acute cerebral infarction was analyzed with the Pearson method,and binary Logistic regression analysis was used to screen the related risk factors affecting cognitive dysfunction in patients with acute cerebral infarction.Results The levels of serum occluding,ZO-1 and MoCA score in cognitive dysfunction group were significantly lower than those in normal cognitive function group(all P<0.05).The levels of serum occludin and ZO-1 in patients with acute cerebral infarction were positively correlated with MoCA score(r=0.614,P=0.000;r=0.607,P=0.000).Multivariate Logistic regression analysis showed that diabetes mellitus was a risk factor for cognitive dysfunction in patients with acute cerebral infarction(P=0.009),and the levels of occludin and ZO-1 were the protective factors of cognitive dysfunction in patients with acute cerebral infarction(P=0.007,P=0.013).Conclusion The serum levels of occludin and ZO-1 in patients with acute cerebral infarction complicated with cognitive dysfunction were low,which may be a protective factor for acute cerebral infarction complicated with cognitive dysfunction,and diabetes is a risk factor.
