儿童臂丛神经炎的临床特点(附4例报告)

Clinical characteristics of brachial plexus neuropathy in children(report of 4 cases)

目的探讨儿童臂丛神经炎的临床特点。方法回顾性分析4例儿童臂丛神经炎患儿的临床资料。结果本组4例臂丛神经炎患儿中男性多见,病前均有感染病史。临床以急性或亚急性起病,表现为单侧上肢的肌无力,累及近端肌肉为主。EMG以臂丛神经上干受损为主,主要表现为复合运动动作电位波幅降低和失神经。予甲泼尼龙及丙种球蛋白静脉注射治疗,随访1年,4例患儿基本完全康复,但遗留不同程度的肌肉萎缩。结论多数臂丛神经炎患儿经免疫治疗后好转,但绝大部分完全康复需要2年以上的时间。

Objective To explore the clinical characteristics of children with brachial plexus neuritis.Methods The clinical data of 4 children with brachial plexus neuritis were retrospectively analyzed.Results Among the 4 cases in this group,most of them were boys,who had a history of infection before the disease.Acute or subacute onset,clinical manifestation was myasthenia of unilateral upper limb,mainly involved the proximal muscle.EMG showed the main damage was in the upper trunk of the brachial plexus,and compound motor action potential showed reduced amplitude and denervation.With methylprednisolone and gamma globulin intravenous therapy and follow-up for 1 year,4 patients recovered almost completely,but left different degrees of muscle atrophy.Conclusion Most patients with brachial plexus neuritis improved after immunotherapy,but most of them need more than 2 years to complete recovery.
