细胞因子NF-κB、IL-17、IL-6在男性不育症中的临床意义 被引量:7

Clinical significance of NF-κB, IL-17 and IL-6 in male infertility

摘要 目的:检测不育症男性患者精浆核因子κB(NF-κB)、白细胞介素-17(IL-17)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)以及生殖道解脲支原体(UU)的含量,探讨其与不育症的关系。方法:抽取2015年8月至2018年6月在连云港市第二人民医院就诊的男性不育症患者53例作为观察组,根据UU检测结果分为UU阳性不育组(30例)和UU阴性不育组(23例)。另抽取同期于医院体检且过去2年内有生育史的健康男性27例为对照组。采用硝酸还原酶法检测精浆一氧化氮(NO)含量,流式细胞仪微球捕获芯片技术(CBA)检测精浆NF-κB、IL-17、IL-6、TNF-α含量,脲酶显色法检测UU含量,并结合计算机辅助精子分析系统对精液常规参数进行分析。结果:UU阳性不育组精浆NO、NF-κB、IL-17、IL-6和TNF-α浓度高于UU阴性不育组(P<0.01);UU阴性不育组精浆NO、NF-κB、IL-17、IL-6和TNF-α浓度高于对照组(P<0.01)。UU阳性不育组精液pH值、精子浓度、形态正常率、存活率、前向运动率均低于UU阴性不育组和对照组(P均<0.01)。男性精液中NO浓度与NF-κB、IL-17、IL-6、TNF-α浓度呈正相关(R2=0.8264、0.9642、0.9143、0.9530,P均<0.01)。结论:UU感染引起男性精浆NO浓度的增高和异常NF-κB、IL-17、IL-6、TNF-α的表达,各因子间的失衡与男性不育有关,可以作为男性不育的辅助鉴别诊断指标。 Objective To detect the contents of nuclear factor-κB(NF-κB),interleukin-17(IL-17),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and Ureaplasma urealyticum(UU)in seminal plasma of infertile male patients,and to investigate its relationship with infertility.Methods Fifty-three infertile male outpatients from the Second People’s Hospital of Lianyungang from August 2015 to June 2018 were selected as the observation group,and they were divided into UU positive infertility group(30 cases)and UU negative infertility group(23 cases)according to UU test results.In addition,27 healthy men with childbearing history in the past two years were selected as the control group.Nitrate reductase method was used to determine the content of nitric oxide(NO)in seminal plasma,the contents of NF-κB,IL-17,IL-6 and TNF-αin seminal plasma were determined by Flow Cytometric Beads Array(CBA),UU content was determined by urease colorimetry,and computer-aided sperm analysis system was used to analyze the routine parameters of semen.Results The concentrations of NO,NF-κB,IL-17,IL-6 and TNF-αin seminal plasma of UU positive infertility group were higher than those of UU negative infertility group(P<0.01).The concentrations of NO,NF-κB,IL-17,IL-6 and TNF-αin seminal plasma of UU negative infertility group were higher than those of control group(P<0.01).Semen pH value,sperm concentration,normal morphology rate,survival rate and forward motility rate in UU positive infertility group were lower than those in UU negative infertility group and control group(P<0.01).The concentration of NO in semen was positively correlated with the concentrations of NF-κB,IL-17,IL-6 and TNF-α(R2=0.8264,0.9642,0.9143,0.9530;all P<0.01).Conclusions UU infection causes the increase of NO concentration and abnormal expressions of NF-κB,IL-17,IL-6 and TNF-αin the seminal plasma.The imbalance of these factors is related to male infertility,which could be used as an assistant diagnostic index for male infertility.
