血清VCAM-1及TNF-α水平与胫骨骨折患者术后延迟愈合的相关性

Correlation of serum levels of VCAM-1 and TNF-αwith postoperative delayed healing in patients with tibial fracture

目的:研究血清血管细胞粘附分子-1(VCAM-1)及肿瘤坏死因子-α(TNF-α)水平与胫骨骨折患者术后延迟愈合的相关性。方法:回顾性分析河南神火集团职工总医院2019年8月至2020年2月收治的103例胫骨骨折患者的临床资料,统计患者术后血清VCAM-1、TNF-α水平和术后延迟愈合情况,并根据术后延迟愈合发生情况将患者分为延迟组和正常组。分析血清VCAM-1、TNF-α水平与胫骨骨折患者术后延迟愈合的相关性。结果:103例胫骨骨折患者术后延迟愈合12例(11.65%),正常愈合91例(88.35%)。延迟组血清VCAM-1、TNF-α水平高于正常组(P<0.05)。Logistic回归分析结果显示,血清VCAM-1、TNF-α高表达可能是胫骨骨折患者发生术后延迟愈合的影响因素(OR>1,P<0.05)。结论:血清VCAM-1、TNF-α高表达和胫骨骨折患者术后延迟愈合密切相关,临床可早期检测患者术后的血清VCAM-1、TNF-α水平,评估患者发生延迟愈合的风险,以早期采取针对性干预措施。

Objective To study the correlation of serum levels of VCAM-1 and TNF-αwith postoperative delayed healing in patients with tibial fracture.Methods The clinical data of 103 patients with tibial fracture treated in Henan Shenhuo Group General Hospital from August 2019 to February 2020 were retrospectively analyzed.The serum VCAM-1,TNF-αlevels,and occurrence of delayed healing after operation were statistically analyzed.According to the occurrence of delayed healing,the patients were divided into delayed group and normal group.The correlation of serum levels of VCAM-1 and TNF-αwith delayed healing after operation was analyzed.Results Among 103 cases of tibial fracture,12 cases(11.65%)had delayed healing and 91 cases(88.35%)had normal healing.The serum levels of VCAM-1 and TNF-αin the delayed group were higher than those in the normal group(P<0.05).Logistic regression analysis showed that the high expression of serum VCAM-1 and TNF-αmay be the influencing factors of delayed healing in patients with tibial fracture(OR>1,P<0.05).Conclusions The high expression of serum VCAM-1 and TNF-αis closely related to the delayed healing of tibial fracture patients after operation.Clinical can detect the serum VCAM-1 and TNF-αlevels of patients after operation early to evaluate the risk of delayed healing of patients,and take targeted intervention measures early.
