模型分析和调[牙合]法治疗运动型食物嵌塞的临床研究

Clinical study of cast analysis and occlusal adjustment for kinetic food impaction

目的:探究模型分析和调[牙合]法治疗运动型食物嵌塞的效果。方法:对符合纳入标准的96名食物嵌塞老年患者进行口腔检查并取模。根据解剖变异、[牙合]力方向失调、[牙合]力大小失调对运动型食物嵌塞进行分类并制定调[牙合]方案。治疗后按照“完全不塞”、“改善明显”、“轻微改善”、“完全没变化”记录患者主观感受评分,并在一年内进行随访。结果:上颌第一与第二磨牙间发生食物嵌塞的几率最大。治疗当天、1个月、3个月、6个月、12个月后患者主观感受评分分别为1.75,1.85,1.68,1.38,1.27。治疗当天与1月后的评分差异有统计学意义(P<0.05)。而治疗当天、3个月、6个月、12个月后各组评分差异均无统计学意义(P>0.05)。各时间点治疗有效率分别为90.63%,89.58%,84.38%,70.83%,61.46%。结论:调[牙合]是运动型食物嵌塞有效的治疗方法。模型分析有助于制定合理的调[牙合]方案。

Objective:To explore the clinical effect of cast analysis and occlusal adjustment for kinetic food impaction.Methods:96 elderly patients with food impaction who met the inclusion criteria were examined and molded.Classification of kinetic food impaction consisted of anatomy variation,occlusal direction disharmony and occlusal force disharmony.Treatment plan was made according to different classification.Patient perception was recorded as“totally relieved”,“significantly improved”,“slightly improved”and“no difference”after occlusal adjustment.Follow-up.was conducted within one year.Results:Food impaction is most likely to occur between maxillary first and second molars.Patient perception scored 1.75,1.85,1.68,1.38,1.27 respectively at the day immediately,1 month,3 months,6 months and 12 months after occlusal adjustment.There was statistical significance of perception score between the day immediately and 1 month after treatment(P<0.05).While no statistical significance existed among the day immediately,3 months,6 months and 12 months after treatment(P>0.05).Efficiency rate was 90.63%,89.58%,84.38%,70.83%,and 61.46%respectively.Conclusions:Occlusal adjustment is an effective treatment option for kinetic food impaction.Cast analysis is helpful for the formulation of a reasonable adjustment scheme.
