可弯曲支气管镜在儿童中心气道异物中的应用

Use of flexible bronchoscopy for removal of central airway foreign bodies in children

目的回顾性分析儿童中心气道异物病例的临床特点和经可弯曲支气管镜的诊治作用。方法分析近10年我科收治的259例中心气道异物的临床资料和可弯曲支气管镜的诊治作用。结果气道异物易并发肉芽增生,其与气道异物性质相关,异物滞留时间、发病年龄是异物并肉芽增生的独立危险因素。经可弯曲支气管镜1次性异物取出成功率达99.6%,预后良好。结论可弯曲支气管镜是中心气道异物取出安全、有效方法,可作为儿童气道异物治疗的首选。

Objective To retrospectively analyze the clinical characteristics of central airway foreign bodies in children and the value of flexible bronchoscopy for the diagnosis and treatment.Methods The clinical data of 259 children with central airway foreign bodies and value of flexible bronchoscopy in removal of the foreign bodies were reviewed.Results The foreign bodies were frequently accompanied by tissue hyperplastic granulation which was associated to the their nature.The duration of foreign body entrapment and age of onset were independent risk factors for foreign body with granulation.99.6%of the cases were successfully removed by a flexible bronchoscope with one attempt,resulting in favorable prognosis.Conclusion Flexible bronchoscopy is a safe and effective option to remove foreign bodies in the central airway,and can be used as the first choice for treatment of airway foreign bodies.
