电针辅助局麻对Ⅰ型腹股沟疝无张力修补术中镇痛效果的影响

Effect of electroacupuncture assisting local anesthesia on analgesia effect in tension-free herniorrhaphy of type 1 inguinal hernia

目的:观察电针辅助局麻在Ⅰ型腹股沟疝无张力修补术中的镇痛效果。方法:选取Ⅰ型腹股沟疝患者40例,随机分为两组各20例,两组均予手术治疗,其中治疗组予电针联合局麻,对照组予局麻。比较两组术中牵拉反应、术中局麻药用量,以及不同时间点的平均动脉压(MAP)、心率(HR)、疼痛分级。结果:两组患者术中牵拉反应比较,差异无统计学意义(P﹥0.05)。两组局麻药用量及疼痛分级比较,差异有统计学意义(P﹤0.05)。两组MAP和HR比较,T1、T2、T3时间点组间比较,差异均具有统计学意义(P﹤0.05);组内比较,治疗组T1、T2、T3各时间点与T0组内比较,差异均具有统计学意义(P﹤0.05),对照组仅MAP在T3时间点与T0组内比较,差异具有统计学意义(P﹤0.05)。结论:电针辅助局麻在Ⅰ型腹股沟疝无张力修补术中可增强镇痛效果,对患者血压、心率有良好的调节作用。

Objective:To observe the analgesia effect of electroacupuncture assisting local anesthesia in tension-free herniorrhaphy of type 1 inguinal hernia.Methods:40 cases of selected patients with type 1 inguinal hernia were randomly divided into two groups,20 cases in each group.Both groups were treated by operation.In the operation,the treatment group was anaesthetized by electro-acupuncture assisting local anesthesia,while the control group was anaesthetized by simple local anesthesia.The intraoperative traction reaction and intraoperative dosage for local anesthesia as well as mean arterial pressure(MAP),heart rate(HR)and pain grading at different time points were compared between two groups.Results:Compared the intraoperative traction reaction of patients between two groups,the difference was not statistically significant(P﹥0.05).Compared the dosage for local anesthesia and the pain grading between two groups,the differences were statistically significant(P﹤0.05).Compared MAP and HR between two groups,and compared them between two groups at T1,T2 and T3 time points,the differences were statistically significant(P﹤0.05).Compared within the group:compared T1,T2 and T3 each time point respectively with T0 within treatment group,the differences were statistically significant(P﹤0.05);compared MAP only at T3 time point with MAP at T0 within control group,the difference was statistically significant(P﹤0.05).Conclusion:Electroacupuncture assisting local anesthesia can enhance the analgesic effect in tension-free herniorrhaphy of type 1 inguinal hernia,which has a good regulating effect on blood pressure and heart rate of patients.
