盘龙七片联合四维牵引治疗腰椎间盘突出症椎间孔镜术后神经根刺激症状35例临床观察

Clinical observation on 35 cases of nerve roots irritation after transforaminal endoscopic discectomy for lumbar disc herniation treated by Panlongqi Pian(盘龙七片) combined with four-dimensional traction

目的观察盘龙七片联合四维牵引治疗腰椎间盘突出症椎间孔镜术后神经根刺激症状的临床疗效。方法将70例行椎间孔镜术的腰椎间盘突出症患者采用随机数字表法分为治疗组和对照组,各35例。2组均行椎间孔镜术摘除髓核,术后治疗组采用盘龙七片联合四维牵引治疗,对照组采用甘露醇注射液静脉滴注及甲钴胺片口服治疗。2组均治疗4周,比较2组治疗前后日本骨科协会评估治疗(JOA)评分、Oswestry功能障碍指数(ODI)评分。结果治疗后2组JOA评分均明显升高,ODI评分均明显降低,与同组治疗前比较差异有统计学意义(P<0.05),且治疗组对JOA、ODI评分的改善作用明显优于对照组(P<0.05)。结论盘龙七片联合四维牵引可明显改善腰椎间盘突出症椎间孔镜术后神经根刺激症状,有利于神经功能的恢复,值得临床推广应用。

Objective To observe the clinical efficacy of Panlongqi Pian( 盘龙七片) combined with four-dimensional traction in the treatment of nerve roots irritation after transforaminal endoscopic discectomy for lumbar disc herniation( LDH). Methods Seventy patients underwent transforaminal endoscopic discectomy for LDH were divided into treatment group and control group according to random number table method,with 35 cases in each group. The 2 groups both underwent transforaminal endoscopic resection of nucleus pulposus of intervertebral disc.After surgery,the treatment group was given Panlongqi Pian combined with four-dimensional traction,and the control group was given mannitol injection intravenously and mecobalamin tablets orally. Both groups were treated for 4 weeks. Japanese Orthopedic Association( JOA) scores and Oswestry disability index( ODI) scores were compared between the 2 groups before and after treatment. Results After treatment,JOA scores significantly increased,while ODI scores significantly decreased in the 2 groups,and the difference was statistically significant compared with those in the same group before treatment( P<0.05),and the improvement effect of the treatment group on all indicators was significantly better than that of the control group( P<0.05). Conclusion Panlongqi Pian( 盘龙七片) combined with four-dimensional traction can markedly improve JOA scores and ODI scores of nerve roots irritation after transforaminal endoscopic discectomy for LDH,which deserves clinical promotion and application.
