基于复合权重因子对比剂注射方法的下肢CTA成像优化研究 被引量:3

Optimization Research of New Injection Projection for CT Angiography of Lower Extremity Based on Compound Body Weight Factor

摘要 目的探讨基于复合权重因子(CWF)对比剂注射新方案对下肢动脉CT血管造影(CTA)图像质量的优化研究。方法前瞻性搜集60例行下肢CTA检查患者的资料,将2019年1月至2019年3月期间的30例患者作为对照组[传统注射方案:净体重法(TBW)],将2019年4月至2019年6月期间的30例患者作为实验组(改良注射方案:根据CWF法计算对比剂总量和流率)。采用自定标准、盲式评估方法对两种方案的图像质量以及安全性进行主、客观评价,并通过SAS软件对数据进行统计学分析。结果相比于对照组,实验组动脉CT值达到标准例数较多,差异有统计学意义(χ^(2)=36.9,P<0.05);实验组的图像质量主观评分更高,差异有统计学意义(χ^(2)=5.8,P<0.05);实验组的静脉污染影响更小,差异具有统计学意义(P<0.05);实验组的辐射剂量更低,差异有统计学意义(t=2.75,P<0.05)。结论基于CWF的新注射方案提高了下肢CTA的图像质量和血管内CT值的均匀性,降低了患者辐射剂量,为临床诊断下肢血管性疾病提供了很大的价值。 Objective To explore the optimization of the image quality of CT angiography(CTA) of lower extremity arteries based on a new program of composite weighting factor(CWF) contrast medium injection. Methods Prospective collection of CT angiography data of 60 patients undergoing CTA examination of lower extremities. 30 patients from January2019 to March 2019 served as control group [Traditional injection regimen: Net weight method( TBW) ],April 2019 Thirty patients from the period to June 2019 are the experimental group( modified injection protocol: calculate the total amount of contrast agent and flow rate according to the CWF method). The image quality and safety of the two schemes were subjectively and objectively evaluated by using self-defined standards and blind evaluation methods,and the data was statistically analyzed through the Sas software. Results Compared with the control group,the experimental group’s arterial CT value reached the standard number of cases on average,and the difference was statistically significant( χ2= 36. 9,P <0. 05);the experimental group had a higher subjective score for image quality,and the difference was statistically significant( χ2= 5. 8,P < 0. 05);the effect of intravenous pollution in the experimental group was smaller,and the difference was statistically significant( P < 0. 05);the radiation dose in the experimental group was lower,and the difference was statistically significant( t = 2. 75,P < 0. 05). Conclusion The new injection protocol based on CWF improves the image quality of lower extremity CTA imaging and the uniformity of intravascular CT values,reduces the patient’s radiation dose,and provides great value for clinical diagnosis of lower extremity vascular diseases.
