摘要
目的探讨无线整合型MRI放大器(WAND)在椎动脉横突孔前段血管壁成像的临床应用价值。方法对17例患者分别行头颈部联合线圈、WAND耦合头颈联合线圈模式头颈部一体化高分辨率血管壁成像,扫描范围从颅顶至胸廓入口层面。扫描结束之后测量C7椎体上缘水平椎动脉横突孔前段管壁外侧壁、内侧壁相同位置的信号强度,计算图像信噪比(SNR)和对比噪声比(CNR),对比两种方案的图像质量,组内数据之间的比较采用配对样本t检验,组间数据比较采用独立样本t检验,计量数据采用Mann-Whitney U秩和检验。结果通过WAND耦合头颈线圈采集获得的图像椎动脉横突孔前段外侧壁SNR 31.9±2.4,内侧壁SNR 36.2±2.9,头颈联合线圈采集获得的图像椎动脉横突孔前段外侧壁SNR 35.2±3.8,内侧壁SNR 35.1±3.7,P值均>0.05,两种图像采集方法获得的图像在对椎动脉横突孔前段区域的图像SNR差异无统计学意义;通过WAND耦合头颈线圈采集获得的图像椎动脉横突孔前段外侧壁CNR为7.1±1.3,内侧壁CNR为6.8±1.5,对比头颈联合线圈对椎动脉横突孔前段外侧壁CNR 43.3±3.0,内侧壁CNR 39.0±3.4,P值均<0.05,其差异有统计学意义。结论 WAND耦合线圈通过提高图像CNR来提高椎动脉横突孔前段血管壁图像质量,从而提升对椎动脉横突孔前段血管壁潜在病变显示的敏感性。
Objective To discuss the clinical feasibility and application of high-resolution imaging of the vertebral artery V1 segment using wireless amplified NMR detector(WAND). Methods Seventeen volunteers were enrolled and underwent integrated head-thoracic vascular wall magnetic resonance imaging(MRI) using head-neck combined coil and WAND coupled head-neck combined coil.The imaging protocol was 3 D-CUBE.Subjective scoring and objective evaluation of the same position images of the two programs were compared.Paired sample t test was used to compare data within the group, independent sample t test was used for comparison of data between groups, and Mann Whitney U rank sum test was used for measurement data. Results By WAND coupling of head and neck coil collected images of the hole in vertebral artery wall signal-to-noise ratio 31.9±2.4,inside wall signal-to-noise ratio 36.2 ±2.9,head and neck joint coil collected images of the hole in vertebral artery wall signal-to-noise ratio(35.2±3.8) mm, inside wall signal-to-noise ratio 35.1±3.7,P values were greater than 0.05,the two methods of image acquisition images obtained on the hole in the front area vertebral artery image signal-to-noise ratio difference was not significant;Through WAND coupling of head and neck coil collected images of the hole in vertebral artery wall contrast to noise ratio was 7.1±1.3,inside wall contrast to noise ratio was 6.8±1.5,compared with head and neck joint coil to the hole in vertebral artery wall contrast noise ratio,(43.3±3.0) mm, inside wall contrast noise ratio 39.0±3.4,P values were less than 0.05,the difference was statistically significant. Conclusion WAND coupled coil scanning improves the imaging quality of V1 segment of vertebral artery.The using of WAND-coupled coil in head-neck integrated imaging of vessel wall has the potential to improve the diagnosis of V1 segment of vertebral artery.
