摘要
目的探讨血清中抗核抗体对体外受精-胚胎移植结局的影响及抗核抗体的表达与体外受精-胚胎移植的结局的关系,为体外受精-胚胎移植结局的分析提供科学依据。方法采用多重微球流式免疫荧光技术对200例反复种植失败的患者,检测其血清中总抗核抗体及抗核抗体中抗SSA、抗SSB、抗Sm、抗RNP、抗Scl-70、抗Jo-1、抗CentromereB、抗Histone、抗ds DNA的IgG抗体检测的含量,检出抗核抗体阳性组41例,阴性组159例,比较两组体外受精-胚胎移植后的促排卵和胚胎发育情况。结果两组患者的窦卵泡数、基础内分泌、绒毛膜促性腺激素(HCG)日内分泌、HCG日内膜厚度、成熟卵、总胚胎数和受精率差异均无统计学意义,优胚数、优胚率及流产率两组比较差异有统计学意义。结论抗核抗体(ANA)阳性可能影响体外受精-胚胎移植后的结局,主要表现为患者的优胚数和优胚率会降低,流产率增高,检测其水平将有助于分析其病因并给予恰当的治疗。
Objective To study the effect of serum antinuclear antibodies on the outcome of in vitro fertilization-embryo transfer, and to discuss the relationship between the expression of antinuclear antibodies and the outcome of in vitro fertilization-embryo transfer, so as to provide scientific basis for the analysis of the outcome of in vitro fertilization-embryo transfer. Methods Multiple microsphere flow immunofluorescence technique was used to detect the contents of anti-SSA, anti-SSB, anti-SM, anti-RNP, anti-SCL-70, anti-JO-1, anti-CentromereB, anti-Histone and anti-dsDNA IgG antibodies in serum of 200 patients with repeated implantation failure. Forty-one patients in the positive group and 159 patients in the negative group were detected. Ovulation induction and embryo development after in vitro fertilization-embryo transfer were compared between the two groups Results There was no statistical significance in the number of sinus follicles, basic endocrine, HCG dayli endocrine, endometrial thickness, mature eggs, total number of embryos and fertilization rate in the two groups, but there was statistical significance in the number of high quality embryos, high quality embryo rate and abortion rate in the two groups. Conclusion ANA positivity may affect the outcome after in vitro fertilization-embryo transfer, which is mainly reflected in the decrease of the number and rate of embryos and the increase of abortion rate. Detection of the level of embryos will help to analyze the etiology and give appropriate treatment.
