常规彩超联合超声弹性成像对乳腺肿物的诊断价值研究

Study on the diagnostic value of conventional color ultrasound combined with ultrasonic elastography in breast masses

目的探究常规彩超联合超声弹性成像对乳腺肿物的诊断价值。方法选取2018年7月至2020年8月因乳腺肿物于本院就诊并行手术治疗的100例患者作为研究对象。所有患者均行常规彩超及超声弹性成像检查,以病理结果作为诊断金标准,分析病理结果与常规彩超、超声弹性成像及联合检查结果,并比较常规彩超、超声弹性成像及联合检查对乳腺肿物的诊断价值。结果100例患者共检出108个病灶,经病理结果确诊,其中良性病灶71个,恶性病灶37个;常规彩超检查共检出良性病灶62个,恶性病灶46个;超声弹性成像检查检出良性病灶66个,恶性病灶42个;联合检查共检出良性病灶69个,恶性病灶39个。联合检查在乳腺肿物诊断中灵敏度、特异度、准确度、阳性预测值及阴性预测值均高于常规彩超、超声弹性成像检查,差异有统计学意义(P<0.05)。结论常规彩超联合超声弹性成像在乳腺肿物中的诊断效能高于单项检查,有利于提高临床鉴别诊断乳腺肿物良恶性的准确性,为后续治疗提供指导。

Objective To investigate the diagnostic value of conventional color ultrasound combined with ultrasonic elastography in breast masses.Methods 100 patients with breast masses who underwent surgery in our hospital from July 2018 to August 2020 were selected as the research subjects.All patients underwent conventional color ultrasound and ultrasonic elastography.The pathological results were taken as the gold standard for diagnosis,and the pathological results,conventional color ultrasound,ultrasonic elastography and combined examination were analyzed.The diagnostic value of conventional color ultrasound,ultrasonic elastography and combined examination in breast masses were compared.Results A total of 108 lesions were detected in 100 patients,71 benign lesions and 37 malignant lesions were confirmed by pathology;62 benign lesions and 46 malignant lesions were confirmed by conventional color ultrasound;66 benign lesions and 42 malignant lesions were confirmed by ultrasonic elastography;69 benign lesions and 39 malignant lesions were confirmed by combined examination.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined examination in diagnosis of breast masses were higher than those of conventional color ultrasound and ultrasonic elastography,the difference was statistically significant(P<0.05).Conclusion Conventional color ultrasound combined with ultrasonic elastography in diagnosis of breast masses has higher diagnostic efficiency than single examination,which can improve the accuracy of clinical differential diagnosis of benign and malignant breast masses,and can provide guidance for subsequent treatment.
