肾脏昼夜节律与高血压关系的研究进展

摘要 许多生理功能均表现出很大的昼夜节律波动,尤其是在肾脏中变现更为明显。肾脏昼夜节律与时钟基因关系密切,肾血浆流量、肾小球滤过率以及肾脏对电解质的重吸收和分泌过程均已显示出显著的昼夜节律性。此外,肾脏昼夜节律与血压调控密切相关,且肾脏昼夜节律紊乱与高血压、糖尿病、慢性肾脏病、肾纤维化以及肾结石等疾病的发展相互关联。肾脏作为体内重要的排泄器官,肾脏昼夜节律的波动可能会干扰各种药物的药动学或药效学,因此在高血压患者的诊治过程中需考虑昼夜节律变化对血压带来的影响。未来对血压与肾功能昼夜节律和时钟基因的进一步深入研究,可能会为高血压患者的个体化治疗提供新方向。 Numerous physiological functions show considerable circadian rhythm oscillations,especially in the kidneys.The renal circadian rhythm is closely related to clock genes.Renal plasma flow,glomerular filtration rate,and the process of reabsorption and secretion of electrolytes by the kidneys have all shown significant circadian rhythms.Besides,the renal circadian rhythm is related to the regulation of blood pressure,and the disruption of renal circadian rhythm is associated with the development of hypertension,diabetes,chronic kidney disease,renal fibrosis,and kidney stones.The kidney is an important excretion organ in the body,and disorders of renal function rhythm may interfere with the pharmacokinetics or pharmacodynamics of various drugs.Therefore,it is necessary to consider the influence of circadian rhythm on blood pressure during the diagnosis and treatment of hypertensive patients.In the future,further research on blood pressure and renal function circadian rhythm and clock genes may provide new directions for individualized treatment of hypertensive patients.
