孕中期超声检查胎儿附属物异常的诊断及与母婴结局的关系研究

Study on the diagnosis of fetal appendage abnormalities by ultrasound examination at 13~28 weeks of pregnancy and its relationship with maternal and infant outcomes

目的探究孕13~28周超声检查胎儿附属物异常的诊断及与母婴结局的关系。方法选取2014年7月~2019年10月于我院产前检查并发现胎儿附属物异常的23例孕妇为观察组,另选同期50例产前检查的健康孕妇为对照组,对两组孕妇脐动脉、大脑中动脉血流参数、母婴结局进行对比。结果孕13~28周超声检查结果显示,23例胎儿附属物异常孕妇均为帆状胎盘合并血管前置,准确率100.00%,羊水异常15例(65.22%),其中羊水过多10例,羊水过少5例;两组孕妇脐动脉、大脑中动脉血流参数、母婴结局相比较,差异有统计学意义(P<0.05)。结论孕13~28周超声检查能够获得准确的胎儿附属物异常诊断结果,为明确其与母婴结局的关系、今后临床积极的干预提供可靠的参照依据。

Objective To study the diagnosis of fetal appendage abnormalities from 13~28 weeks of pregnancy and its relationship with maternal and infant outcomes.Methods Twenty-three pregnant women who had undergone birth inspection in our hospital from July 2014 to October 2019 and found abnormal fetal appendages were selected as the observation group,and 50 healthy pregnant women who had undergone birth examination during the same period were selected as the control group.The parameters of middle cerebral artery blood flow and maternal and infant outcomes were compared.Results The results of ultrasound examination at 13~28 weeks of pregnancy indicated that 23 pregnant women with abnormal fetal appendages were sail-shaped placenta with vascular preposition,with an accuracy rate of 100.00%,and 15 cases(65.22%)were abnormally detected.There were 10 cases of polyhydramnios and 5 cases of oligohydramnios.There were statistically significant differences in blood flow parameters and maternal and infant outcomes between the two groups of pregnant women's umbilical artery and middle cerebral artery(P<0.05).Conclusion Ultrasound examination at 13~28 weeks of pregnancy can obtain accurate diagnosis of abnormal fetal appendages,and provide a reliable reference for clarifying its relationship with maternal and infant outcomes and future clinical intervention.
