下肢静脉曲张患者聚桂醇泡沫硬化剂治疗的有效性及对患者血清TNF-α、IL-6水平的影响

Effectiveness of lauryl alcohol foam sclerotherapy for patients with varicose veins of the lower extremities and its effect on the levels of serum TNF-αand IL-6

目的探讨下肢静脉曲张患者聚桂醇泡沫硬化剂治疗的有效性及对患者血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)水平的影响。方法选取130例下肢静脉曲张患者,按照随机数字法进行分组,研究组、对照组各65例,分别采用大隐静脉高位结扎+剥脱术与用超声引导下聚桂醇泡沫硬化剂治疗,对比两组患者临床疗效。结果研究组患者住院费用、术中出血、住院时间、手术时间明显小于对照组(t分别=3.76、21.54、5.29、18.44,P均<0.05);两组患者治疗前循环内皮细胞计数(CEC)、内皮素-1(ET-1)、TNF-α、IL-6、一氧化氮(NO)水平比较,差异均无统计学意义(t分别=0.78、1.65、-1.01、0.73、-1.52,P均>0.05),研究组治疗后CEC、ET-1、TNF-α、IL-6、NO改善较对照组明显,差异均有统计学意义(t分别=33.87、9.65、21.67、8.69、-2.72,P均<0.05);研究组并发症发生率为4.62%明显低于对照组15.38%,差异有统计学意义(χ2=4.19,P<0.05)。结论聚桂醇泡沫硬化剂治疗下肢静脉曲张效果较好,明显改善了血管内皮功能及血清炎症因子水平,减少了并发症。

Objective To explore the effectiveness of lauryl alcohol foam sclerotherapy for patients with varicose veins of the lower extremities and its effect on the serum tumor necrosis factorα(TNF-α)and interleukin-6(IL-6)levels.Methods Totally 130 patients with varicose veins of the lower extremities were selected and grouped according to the random number method,with 65 cases in the study group and 65 cases in the control group.The patients in the control group were treated with high ligation combined with ablation of great saphenous vein,and the patients in the study group were received ultrasonic guided lauromacromacrol foam sclerotherapy.The clinical efficacy of the two groups was com-pared.Results The hospitalization cost,intraoperative bleeding volume,hospitalization time,and operation time of the study group were significantly less than those of the control group(t=3.76,21.54,5.29,18.44,P<0.05).The levels of CEC,ET-1,TNF-α,IL-6,and NO before treatment between the two groups were not significantly different(t=0.78,1.65,-1.01,0.73,-1.52,P>0.05).After treatment,the levels of CEC,ET-1,TNFα,IL-6 and NO of the study group were significantly improved when compared to those of the control group(t=33.87,9.65,21.67,8.69,-2.72,P<0.05).The complication rate of patients in the study group was 4.62%,which was significantly lower than 15.38%in the control group(χ2=4.19,P<0.05).Conclusion Lauryl alcohol foam sclerotherapy has better effects in treatment of lower extre-mity varicose veins,obviously improve the endothelial function and serum levels of inflammatory cytokines,and reduce the complications.
