侧切减压切口联合部分内括约肌切断在环状混合痔术中的疗效观察

Effect of Lateral Incision&Decompression Combined with Partial Amputation of Inner-Sphincter in the Treatment of Circumferential Mixed Hemorrhoids

为探讨侧切减压切口联合部分内括约肌切断在环状混合痔术中的应用效果,将60例环状混合痔患者随机分成治疗组和对照组,各30例。治疗组在外剥内扎术中采用侧切减压切口联合部分内括约肌切断治疗,对照组采用传统外剥内扎术治疗,观察比较两组临床效果、并发症及满意度情况。结果显示,两组临床总有效率均为100%,差异无统计学意义(P>0.05),但治疗组水肿、疼痛、便血和肛门狭窄的发生率均低于对照组,差异有统计学意义(P<0.05)。治疗组患者满意度评分为(92.3±15.2)分,显著高于对照组的(81.6±10.7)分,差异有统计学意义(P<0.05)。结果表明,侧切减压切口联合部分内括约肌切断术能降低肛管压力,有效预防术后并发症的发生,从而减轻术后疼痛及出血,临床疗效满意。

This study was to investigate the effect of lateral incision&decompression(LID)plus partial amputation of inner-sphincter in the treatment of circumferential mixed hemorrhoids(CMH),randomly divided 60CMH patients into treatment group(30cases,received external dissection&internal ligation with LID plus partial amputation of inner-sphincter)and control group(30cases,simply external dissection&internal ligation);then,observed and analysed both groups'clinical effect,complication and recurrence status.As results,in clinical effective rate there was no statistical difference between the two groups(P>0.05);but,in the incidence of edema,pain,hematochezia,and anal stenosis,and in the score ratings on patients'satisfactory degree treatment group was respectively and significantly lower,and higher[(92.3±15.2)point vs(81.6±10.7)points]than control group(P<0.05,all).Results show that LID plus partial amputation of inner-sphincter can decline anal pressure,and effective prevent postoperative complications happing,so that relieve postoperative pain and bleeding,thus with satisfactory clinical effect.
