化痰通络方联合醒脑开喑法针刺治疗脑中风后言语不利疗效研究

Efficacy of Huatan Tongluo Recipe Combined with ″Xingnaokaiyin″ Acupuncture in the Treatment of Patients with Speech Impairment After Stroke

目的探讨化痰通络方联合醒脑开喑法针刺治疗脑中风后言语不利的疗效。方法选取河北省衡水市第四人民医院2018年3月至2020年3月收治的脑中风后言语不利患者97例,随机分为研究组(49例)和对照组(48例)。两组患者均行常规治疗,研究组患者加用化痰通络方联合醒脑开喑法针刺治疗,均治疗4周。结果研究组患者的总有效率为87.76%,显著高于对照组的70.83%(P<0.05);研究组患者的中医证候舌质僵硬、口角歪斜、构音障碍评分均显著低于对照组(P<0.05),语言功能(自发谈话、理解、复述、命名)评分、血清学指标(脑源性神经营养因子、神经营养因子3、神经生长因子)水平均显著高于对照组(P<0.05);研究组和对照组不良反应发生率相当(14.29%比8.33%,P>0.05)。结论化痰通络方联合醒脑开喑法针刺治疗脑中风后言语不利疗效良好,能改善患者的中医证候,恢复语言功能,提高脑神经功能,且治疗安全性良好。

Objective To investigate the efficacy of Huatan Tongluo Recipe combined with″Xingnaokaiyin″acupuncture in the treatment of patients with speech impairment after stroke.Methods A total of 97 patients with speech impairment after stroke admitted to the No.4 People’s Hospital of Hengshui from March 2018 to March 2020 were selected and randomly divided into the study group(49 cases)and the control group(48 cases).The patients in the two groups were given conventional treatment,on this basis,the patients in the study group were treated with Huatan Tongluo Recipe combined with″Xingnaokaiyin″acupuncture.Both groups were treated for four weeks.Results The total effective rate of the study group was 87.76%,which was significantly higher than 70.83%of the control group(P<0.05).The scores of traditional Chinese medicine(TCM)syndromes of tongue stiffness,angular deviation and dysarthria in the study group were significantly lower than those in the control group(P<0.05),while the scores of language function(spontaneous conversation,understanding,retelling and naming),and the levels of serological indexes[brain-derived neurotrophic factor(BDNF),neurotrophic factor-3(NTF-3)and nerve growth factor(NGF)]in the study group were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions in the study group was similar to that in the control group(14.29%vs.8.33%,P>0.05).Conclusion Huatan Tongluo Recipe combined with″Xingnaokaiyin″acupuncture is effective and safe in the treatment of patients with speech impairment after stroke,which can improve the patients’TCM syndromes,restore the language function,and improve the brain nerve function.
