184例妊娠合并糖尿病产妇分娩的临床处理经验总结

A Summary of the Clinical Management Experience of 184 Pregnant Women with Diabetes during Childbirth

目的观察分析184例妊娠合并糖尿病产妇分娩结局。方法选取2019年1月—2020年1月该院收治的184例妊娠合并糖尿病产妇为观察组,选取同期100名血糖正常产妇为对照组,观察比较两组产妇一般资料、分娩方式及结局差异,总结其临床处理经验。结果观察组患者剖宫产概率高于对照组,差异有统计学意义(P<0.05),产妇妊娠高血压、羊水过多及并发症总发生率高于对照组,差异有统计学意义(P<0.05),围生儿在早产、巨大儿、胎儿窘迫、高胆红素血症及并发症总发生率均高于对照组,差异均有统计学意义(P<0.05)。结论妊娠合并糖尿病产妇产前发生高血压,新生儿易出现高胆红素血症等风险明显增加,由于妊娠与糖尿病的相互影响,易使病情加重,如处理不当,可出现较多严重并发症,增高母婴病死率。

Objective To analyze the delivery outcomes of 184 pregnant women with diabetes mellitus.Methods Selected 184 pregnant women with diabetes mellitus admitted to the hospital from January 2019 to January 2020 as the observation group,and 100 women with normal blood glucose were selected as the control group.The differences in general information,delivery methods and outcomes of the two groups were observed and compared,and their clinical treatment experience was summarized.Results The probability of cesarean section in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The total incidence of pregnancy-induced hypertension,polyhydramnios and complications was higher than in the control group,the difference was statistically significant(P<0.05).The total incidence of fetal distress,hyperbilirubinemia and complications were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion High blood pressure in pregnant women with diabetes mellitus,and neonatal asphyxia are prone to significantly increase the risk hyperbilirubinemia.Due to the mutual influence of pregnancy and diabetes,the disease is easily aggravated.If not handled properly,more serious complications may occur,increase maternal and infant mortality.
